Apps auf Rezept: Warum das deutsche Modell funktioniert und trotzdem niemanden ernährt
Seit 2019 kann ein Arzt in Deutschland eine App verschreiben, und die Krankenkasse zahlt. Weltweit gibt es das sonst nirgends. Sechs Jahre später verlassen die Anbieter den Markt — wegen einer einzigen Regel, die kaum jemand kennt. Dieser Bericht erklärt sie von Grund auf.

Stellen Sie sich vor, Sie gehen mit Rückenschmerzen oder einer Depression zum Arzt. Statt einer Tablette oder einer Überweisung bekommen Sie ein Rezept für eine Smartphone-App. Sie reichen es bei Ihrer Krankenkasse ein, bekommen einen Freischaltcode, laden die App herunter und nutzen sie 90 Tage lang. Die Kasse zahlt. Sie zahlen nichts.
Genau das gibt es in Deutschland seit 2019 — und sonst in keinem anderen Land der Welt in dieser Form. Diese Apps heißen offiziell digitale Gesundheitsanwendungen, abgekürzt DiGA. Umgangssprachlich: „Apps auf Rezept“.
Der Gedanke dahinter ist gut. Auf einen Therapieplatz wartet man in Deutschland im Schnitt fast fünf Monate. Eine App ist sofort da. Wenn sie hilft, überbrückt sie eine echte Lücke.
Die Frage dieses Berichts ist eine andere: Warum wollen immer weniger Unternehmen diese Apps noch bauen? Denn die Zahlen sagen das eindeutig. 2021 gingen 68 Anträge beim zuständigen Bundesinstitut ein. 2025 waren es 23.
Wie das System funktioniert
Wie eine App aufs Rezept kommt — und wie ihr Preis entsteht
Zwei Wege, die parallel laufen: der Weg zum Patienten links, der Weg zum Preis rechts.
Zwei Dinge laufen parallel. Links der Weg zum Patienten: Die App muss erst ein zugelassenes Medizinprodukt sein, dann prüft das Bundesinstitut für Arzneimittel und Medizinprodukte — kurz BfArM — den Antrag. Fällt die Prüfung positiv aus, landet die App in einem öffentlichen Verzeichnis und darf verordnet werden.
Rechts der Weg zum Preis. Und der ist der eigentliche Gegenstand dieses Berichts. Im ersten Jahr legt der Hersteller den Preis selbst fest. Danach verhandeln die Krankenkassen mit ihm. Werden sie sich nicht einig, entscheidet eine Schiedsstelle. Der so festgelegte Preis gilt rückwirkend — auch für alle Verordnungen des ersten Jahres.
Die Begriffe, die immer wieder vorkommen
DiGA — eine App, die ein Arzt verschreiben kann und die Krankenkasse bezahlt. BfArM — die Bundesbehörde, die über die Aufnahme entscheidet. GKV-Spitzenverband — der Dachverband der gesetzlichen Krankenkassen; er verhandelt den Preis für alle Kassen gemeinsam. Vergütungsbetrag — der ausgehandelte Preis, den die Kasse ab dem zweiten Jahr zahlt. Erprobung — eine vorläufige Aufnahme für zwölf Monate. Der Hersteller verdient bereits Geld und muss den Nutzen erst danach belegen. Positiver Versorgungseffekt — der gesetzlich geforderte Nachweis, dass die App etwas bringt. Entweder medizinisch messbar, oder als Verbesserung im Behandlungsablauf. Freischaltcode — was der Patient von der Kasse bekommt, um die App zu aktivieren. Medizinprodukt / MDR — Software, die bei Diagnose oder Therapie hilft, gilt rechtlich als Medizinprodukt und braucht eine Zulassung nach der EU-Medizinprodukteverordnung (englisch Medical Device Regulation). Randomisierte Studie (RCT) — der Goldstandard des Wirksamkeitsnachweises: Teilnehmende werden per Zufall auf zwei Gruppen verteilt; eine bekommt die Behandlung, die andere nicht. Effektstärke (Cohens d) — ein Maß dafür, wie groß ein Behandlungseffekt ist. Grobe Faustregel: 0,2 klein, 0,5 mittel, 0,8 groß.
Drei Fragen, drei verschiedene Antworten
Dieser Bericht beantwortet drei Fragen getrennt, weil die Evidenz für sie unterschiedlich ausfällt:
Soll ich als Patient oder Arzt darauf setzen? Manchmal ja — dazu Teil 6 und Teil 14.
Soll ich als Gründer so etwas bauen? Fast nie — dazu Teil 5 und Teil 11.
Soll ich als Investor Geld hineinstecken? Nur mit sehr genauem Blick auf eine einzige Vertragsklausel — dazu Teil 5 und Teil 14.
Bevor es um Zahlen geht, eine Unterscheidung, an der die meisten Investitionsentscheidungen in diesem Feld scheitern. „Digital Health“ ist kein Markt, sondern ein Sammelbegriff. Darunter liegen fünf Geschäfte mit völlig unterschiedlicher Ökonomie.

Was das heißt: Wer eine Marktzahl für „Digital Health“ liest und daraus auf das Potenzial einer Therapie-App schließt, vergleicht Praxissoftware mit Rezept-Apps. Das sind verschiedene Welten, und der Unterschied beträgt zwei Größenordnungen. Teil 3 rechnet das vor.
Beginnen wir mit der Zahl, die alles andere einordnet. Die Krankenkassen müssen jährlich berichten, wie viel sie für DiGA ausgeben. Für den gesamten Zeitraum vom 1. September 2020 bis zum 31. Dezember 2025 steht dort: 1,6 Millionen abgegebene Apps und rund 401 Millionen Euro. Für das Jahr 2025 allein: 695.000 Abgaben und 171 Millionen Euro.
Wo im deutschen Digital-Health-Markt tatsächlich Umsatz entsteht
Jahresumsatz beziehungsweise Ausgaben der Krankenkassen, in Millionen Euro.
Das ist der komplette Markt für Apps auf Rezept in Deutschland — nach sechs Jahren, mit gesetzlichem Anspruch, in einem Gesundheitssystem, das jährlich über 300 Milliarden Euro ausgibt.
Zum Vergleich: CompuGroup Medical verkauft Software an Arztpraxen und Kliniken. Kein Patient hat je davon gehört. Der Umsatz 2025: 1,213 Milliarden Euro. Doctolib, die Terminplattform, kam auf 422 Millionen. Beide verkaufen keine Therapie, sondern Arbeitsabläufe — und beide haben nie eine Erstattung beantragt.

Eine einzelne Zahl sagt wenig, solange man sie nicht einordnen kann. Eine DiGA kostet die Kasse im Schnitt 227 Euro für 90 Tage. Eine Sitzung Richtlinienpsychotherapie kostet sie 114,54 Euro. Eine App entspricht also ungefähr zwei Therapiesitzungen — und etwa einem Sechstel dessen, was ein Quartal mit wöchentlicher Therapie kostet.
Aus Sicht der Kasse ist das nicht teuer. Aus Sicht des Herstellers ist es das Problem: Von diesen 227 Euro müssen Entwicklung, eine randomisierte Studie, die Medizinprodukt-Zulassung und der Vertrieb bezahlt werden.
Wie sich die 171 Millionen verteilen
Sehr ungleich. Über den gesamten Zeitraum entfallen 37 Prozent der Nutzungen auf Stoffwechselerkrankungen und 27 Prozent auf psychische Erkrankungen — während psychische Erkrankungen 55 Prozent des Katalogs ausmachen. Eine Handvoll Stoffwechsel-Apps erzeugt mehr Volumen als drei Dutzend Psyche-Apps.
Innerhalb der Stoffwechselgruppe konzentriert sich das Volumen weiter auf einen einzigen Anbieter: Oviva kam 2024 auf 29 Prozent aller Verordnungen und 2025 auf rund 44 Prozent. REPORTED
Teilt man die 171 Millionen schlicht durch 58 gelistete Apps, ergibt das im Schnitt 2,9 Millionen Euro Umsatz pro App und Jahr. Wegen der Konzentration liegt der tatsächliche Wert für fast alle Anbieter deutlich darunter — wie weit darunter, rechnet Teil 7 vor.EIGENE RECHNUNG
AREF SIGNAL Die Marktzahlen, die zitiert werden, messen etwas anderes
Marktforschungshäuser beziffern den deutschen Digital-Health-Markt für 2026 auf rund 11,9 Milliarden US-Dollar. Ein anderer Anbieter nennt für europäische „Digital Therapeutics“ 2,58 Milliarden Dollar für 2025. Ein Land ist also viermal so groß wie ein Kontinent — weil verschiedene Dinge gezählt werden. Die große Zahl enthält Telemedizin, Klinik-IT, vernetzte Geräte und Dienstleistungen.
Keine dieser Zahlen misst den DiGA-Markt. Der reale Umsatz beträgt 171 Millionen Euro — etwa anderthalb Prozent der 11,9 Milliarden. EIGENE RECHNUNG
Wer eine Digital-Health-Marktzahl als Näherung für das Erlöspotenzial einer Therapie-App benutzt, überschätzt es um den Faktor siebzig. Das ist keine Feinheit. Das ist der Unterschied zwischen einem Geschäftsmodell und einem Missverständnis.

Was mit einer verordneten App tatsächlich passiert#
Bevor man über Wirksamkeit streitet, lohnt eine einfachere Frage: Benutzen die Leute die App überhaupt? Die Kassen können das sehen, weil jeder Freischaltcode registriert wird.
Von hundert Rezepten bleiben siebzehn

Von hundert ausgegebenen Codes werden 82 überhaupt eingelöst. 18 landen in der Schublade. Und von denen, die einlösen, holen sich 83 Prozent nach den 90 Tagen kein neues Rezept. REPORTED
Diese 83 Prozent sind keine Abbruchquote. Wer kein Folgerezept holt, kann geheilt sein, das Interesse verloren haben oder schlicht die Indikation hinter sich haben. Das Problem ist: Die Kassen können den Unterschied nicht sehen. Die Nutzungsdaten liegen bei den Herstellern, nicht bei den Zahlern. Eine echte Abbruchquote innerhalb der 90 Tage steht in keiner öffentlichen Quelle.
Das ist eine strukturelle Lücke, keine Recherchelücke — und genau deshalb verlangt der Gesetzgeber ab Oktober 2026 erstmals, dass Hersteller Nutzungsdauer, Nutzungshäufigkeit und Abbruchquoten melden.
Die härtere Vergleichszahl steht im App-Store
Für Gesundheits-Apps außerhalb des Rezeptsystems gibt es unabhängige Nutzungsdaten. Eine Untersuchung von 93 Apps für Angst, Depression und Wohlbefinden, jede mit mindestens 10.000 Installationen, fand: im Mittel 4,0 Prozent täglich aktive Nutzer. Nach 15 Tagen waren noch 3,9 Prozent dabei, nach 30 Tagen 3,3 Prozent.
Diese Zahlen stammen aus dem freien Markt. Ein ärztliches Rezept verbessert die Sache deutlich — die 82 Prozent Aktivierung zeigen das. Aber sie beschreiben die Schwerkraft, gegen die jede Gesundheits-App arbeitet. Und sie erklären, warum zwei Drittel aller jemals gebauten Gesundheits-Apps heute nicht mehr angeboten werden.
Und was passiert mit den Anbietern?
Ein Markt, aus dem sich die Anbieter zurückziehen
Neuanträge beim BfArM und Zusammensetzung des Verzeichnisses.
Von 74 jemals aufgenommenen Apps wurden 16 wieder gestrichen — jede fünfte. Der Grund ist fast immer derselbe: Der Nutzen konnte nicht belegt werden. 34 Apps schafften den Sprung von der Erprobung in die dauerhafte Aufnahme, jede fünfte davon nur mit eingeschränktem Leistungsumfang.
Und die aussagekräftigste Zahl steht auf der Antragsseite: 68 Anträge im Jahr 2021, 23 im Jahr 2025. Ein Rückgang um zwei Drittel in einem Markt, der formal wächst. Die Anbieter stimmen mit den Füßen ab.
Hier liegt der eigentliche Befund dieses Berichts. Das deutsche Modell hat einen Konstruktionsfehler, der nicht theoretisch ist. Er hat nachweislich Unternehmen vernichtet — und nicht die schlechtesten.
Was mit dem Preis passiert, sobald verhandelt wird
Herstellerpreis im ersten Jahr gegen den ausgehandelten Betrag, in Euro je Verordnung.
Der Ablauf steht im Gesetz. Im ersten Jahr setzt der Hersteller den Preis frei. Danach verhandelt der GKV-Spitzenverband mit ihm. Kommt binnen neun Monaten keine Einigung zustande, entscheidet eine Schiedsstelle binnen drei weiteren Monaten. Bis Ende 2025 gab es 40 solcher Preise; etwa jeder dritte wurde von der Schiedsstelle festgesetzt.
Über alle Apps hinweg fällt der Preis dabei von durchschnittlich 544 Euro auf 227 Euro — ein Minus von 59 Prozent. Die größte dokumentierte Einzelreduktion ging von 2.077,40 Euro auf 247,81 Euro.
Das allein wäre harte, aber normale Preisregulierung. Der Konstruktionsfehler ist ein anderer.
Der festgesetzte Preis gilt rückwirkend
Die Preisfalle, einmal durchgerechnet
Beispiel für einen Anbieter mit 1.000 Verordnungen im ersten Jahr.
Rechnen wir es durch. Ein Anbieter startet mit dem Durchschnittspreis von 544 Euro und wird im ersten Jahr 1.000 Mal verordnet. Er nimmt 544.000 Euro ein und baut damit sein Unternehmen auf: Personal, Vertrieb, Weiterentwicklung.
Nach zwölf Monaten wird der Preis auf 227 Euro festgesetzt — und zwar rückwirkend ab dem ersten Verkaufstag. Für die bereits abgerechneten 1.000 Verordnungen gilt jetzt der neue Preis. Die Differenz von 317 Euro je Verordnung muss zurückgezahlt werden: 317.000 Euro, für Umsatz, der längst ausgegeben ist.
Und jetzt der perverse Teil: Je erfolgreicher der Anbieter im ersten Jahr war, desto größer die Rückforderung. Erfolg wird bestraft.
Zwei Insolvenzen, dieselbe Ursache
Newsenselab mit der Migräne-App M-sense war der erste Fall. Die Schiedsstelle senkte den Preis von 219,98 Euro auf 10 Euro — ein Minus von 95 Prozent. Die App verließ im April 2022 das Verzeichnis, im Juli 2022 folgte die Insolvenz.
aidhere mit der Adipositas-App zanadio meldete am 30. Mai 2023 beim Amtsgericht Hamburg Insolvenz an. Die Schiedsstelle hatte den Preis von 499,80 Euro auf 218 Euro gesenkt, rückwirkend bis Oktober 2021. Das Unternehmen erklärte, „der um mehr als die Hälfte reduzierte Vergütungsbetrag und die daraus drohenden Rückzahlungsansprüche“ seien „wirtschaftlich nicht tragbar“. zanadio war zu diesem Zeitpunkt die meistverordnete App Deutschlands mit über 30.000 Nutzern. Die App überlebte — ein anderes Unternehmen kaufte sie —, die Firma nicht.
Der Fall zanadio ging vor Gericht. Das Landessozialgericht Berlin-Brandenburg entschied am 20. Februar 2025, dass der Schiedsspruch „im Wesentlichen durch den weitreichenden Entscheidungsspielraum der Schiedsstelle gedeckt“ war. Nur bei der Erprobungsdauer hob es auf. Revision zum Bundessozialgericht wurde zugelassen; rechtskräftig ist das Urteil nicht. CONFIRMED
Und in die andere Richtung gilt die Regel nicht
Was die Kassen für Apps gezahlt haben, die den Nutzen nie belegten
Ausgaben im jeweils ersten Aufnahmejahr, in Millionen Euro.
Hier wird es asymmetrisch. Wenn ein Hersteller im ersten Jahr zu viel verlangt hat, muss er zurückzahlen. Wenn eine App den Nutzen nie belegt und deshalb gestrichen wird, passiert das Gegenteil. Der Bericht der Krankenkassen formuliert es unmissverständlich:
Für Ausgaben aus dem jeweils ersten Aufnahmejahr einer DiGA besteht auch dann kein Rückzahlungsanspruch für die GKV, wenn die DiGA keinen positiven Versorgungseffekt nachgewiesen hat.

Für die 16 gestrichenen Apps haben die Kassen im jeweils ersten Jahr 7,53 Millionen Euro gezahlt. Zurück bekommen sie davon nichts. Bei 15 der 16 kam es nie zu einem verhandelten Preis — sie verschwanden vorher.
Was das heißt: Das Erprobungsjahr ist eine Einbahnstraße in beide Richtungen. Der Hersteller trägt das volle Preisrisiko nach unten. Die Kasse trägt das volle Nutzenrisiko. Niemand gewinnt an dieser Konstruktion — und deshalb greift die Kritik, die nur eine Seite sieht, immer zu kurz.
Was der Gesetzgeber daraus gemacht hat
Seit dem 1. Januar 2026 müssen mindestens 20 Prozent des Preises erfolgsabhängig sein. Seit dem 1. Februar 2026 gilt eine Pflicht zur laufenden Erfolgsmessung, gestaffelt in drei Stufen: ab Oktober 2026 Nutzungsdaten samt Abbruchquoten, ab April 2028 die Einschätzung der Patienten, ab April 2029 krankheitsspezifische Fragebögen. Beides adressiert die Evidenzlücke. Keines von beiden adressiert die Rückwirkung.
Jetzt die Frage, die am Anfang stehen müsste: Helfen diese Apps den Menschen? Die ehrliche Antwort lautet — vermutlich ja, ein bisschen, und niemand weiß genau wie viel. Das klingt unbefriedigend. Es ist aber präziser als alles, was Hersteller und Kritiker öffentlich behaupten.
Was eine Effektstärke bedeutet
Um Wirkung zu vergleichen, benutzt die Forschung eine Maßzahl namens Effektstärke. Man muss dafür nicht rechnen können, nur eine Faustregel kennen: 0,2 ist klein, 0,5 mittel, 0,8 groß. Anschaulicher: Bei einer Effektstärke von 0,2 geht es einem durchschnittlichen Behandelten besser als 58 von 100 Unbehandelten. Bei 0,8 sind es 79 von 100.
Der entscheidende Punkt ist aber nicht die Zahl selbst, sondern womit verglichen wurde.
Wie stark die Wirkung schrumpft, wenn die Vergleichsgruppe etwas bekommt
Effektstärken digitaler psychologischer Anwendungen, nach Art der Vergleichsgruppe.
Vergleicht man eine App mit einer Warteliste — also mit Menschen, die gar nichts bekommen —, sieht sie gut aus. Vergleicht man sie mit einer Gruppe, die irgendetwas anderes bekommt, schrumpfen die Effekte auf ein Fünftel bis ein Zwanzigstel. Bei Lebensqualität bleibt von 0,35 noch 0,02 übrig, also praktisch nichts.
Und jetzt der wichtige Satz: Alle deutschen Zulassungsstudien für DiGA verwenden inaktive Kontrollgruppen. Sie messen also die freundlichere der beiden Zahlen. CONFIRMED
Wie klein die Studien sind
Zwei Evidenzwelten in einem Feld
Zahl der Teilnehmenden in Zulassungsstudien, logarithmisch dargestellt.
Eine unabhängige Auswertung aller 23 Zulassungsstudien zu 21 DiGA kam im August 2025 zu einem harten Urteil. Alle Studien berichteten mittlere bis große Effekte — aber: „All studies were evaluated to have an overall high risk of bias.“ Ausnahmslos alle. Besonders bei der Ergebnismessung (21 von 23 Studien) und bei fehlenden Daten (15 von 23).
Dazu kommt ein Muster, das man kennen sollte: In den Interventionsgruppen brachen im Schnitt 21,7 Prozent ab, in den Kontrollgruppen nur 11,8 Prozent. Wer eine App bekommt, steigt also fast doppelt so oft aus wie wer auf der Warteliste sitzt — und wer aussteigt, wird oft nicht mitgemessen. Das schönt die Ergebnisse in eine bekannte Richtung.
Die stärkste je gemessene DiGA-Wirkung (Effektstärke 1,79) stammt aus der kleinsten Studie mit 56 Teilnehmenden gegen eine Warteliste. Das ist kein Beweis für Manipulation. Es ist das statistisch Erwartbare: Kleine Studien schwanken stärker, und nach oben schwankende Ergebnisse werden eher veröffentlicht.
Die eine Studie, die die Geldfrage stellt
Fast alle Studien fragen: Geht es den Menschen besser? Nur wenige fragen: Ist es das Geld wert? Für die Depressions-App deprexis gibt es beides.
Das Ergebnis, präzise — und warum es meist falsch zitiert wird
In einer randomisierten Studie mit 3.805 Versicherten einer großen Krankenkasse sanken die Gesamtkosten in der App-Gruppe binnen eines Jahres um 1.021 Euro je Patient (minus 32 Prozent). Diese Zahl steht in fast jeder Zusammenfassung.
Was dabei weggelassen wird: In der Vergleichsgruppe sanken die Kosten ebenfalls — um 436 Euro (minus 13 Prozent). Der tatsächliche Unterschied zwischen den Gruppen beträgt also rund 585 Euro, nicht 1.021. Dieser Unterschied war statistisch gerade eben signifikant (p = 0,04).
Und noch etwas: Die Studie untersuchte auch Einzelposten — Arzneimittel, Krankenhaus, Krankengeld. Kein einziger davon unterschied sich signifikant zwischen den Gruppen (p = 0,14 / 0,25 / 0,16). Belastbar ist nur die Gesamtsumme, nicht die Erzählung, welcher Posten die Ersparnis bringt. CONFIRMED
Eine zweite, neuere Modellrechnung stellt die Frage andersherum: Ab welchem Preis wäre eine solche App der Standardbehandlung nicht nur ebenbürtig, sondern überlegen — günstiger und wirksamer? Die Antwort: unterhalb von 20,34 Euro je Quartal. Der heutige durchschnittliche Vergütungsbetrag liegt bei 227 Euro je 90 Tage — etwa das Elffache.CONFIRMED
Fairerweise gehört die Gegenrechnung dazu, und sie stammt aus einem anderen Krankheitsgebiet. Für digitale Anwendungen bei Rückenschmerzen ermittelte eine gesundheitsökonomische Modellierung Kosten von rund 5.486 Euro je gewonnenem gesunden Lebensjahr — aus gesellschaftlicher Perspektive, also inklusive Arbeitsausfall. Das liegt weit unter dem, was Gesundheitssysteme üblicherweise zu zahlen bereit sind. Nach diesem Maßstab wäre die Anwendung günstig. CONFIRMED
Beide Rechnungen sind seriös und widersprechen sich trotzdem. Der Grund ist banal und wichtig: Die eine fragt „spart die Krankenkasse Geld?“, die andere fragt „ist der Gesundheitsgewinn seinen Preis wert — für die Gesellschaft?“. Wer den Arbeitsausfall mitrechnet, kommt zu einem völlig anderen Ergebnis als wer nur das Kassenbudget sieht.
Deutschland beantwortet offiziell keine der beiden Fragen. Im DiGA-Verfahren wird gar keine gesundheitsökonomische Analyse verlangt. In Frankreich und Großbritannien ist sie Standard. CONFIRMED
Dieselbe App, drei Länder, drei Urteile
Wie unsicher der Boden ist, zeigt ein einzelnes Produkt. deprexis ist in Deutschland dauerhaft gelistet. In der Schweiz wird es seit dem 1. Juli 2026 erstattet. Und der englische NHS stufte es 2024 als nicht konform mit seinen Bewertungskriterien ein, mit der Begründung, wesentliche Inhalte deckten sich nicht mit den NICE-Leitlinien. CONFIRMED
Dasselbe Produkt, dieselbe Datenlage, drei entgegengesetzte Entscheidungen. Wer behauptet, die Evidenzfrage sei geklärt — in die eine oder die andere Richtung —, hat sie nicht gelesen.
Wenn über „den DiGA-Markt“ gesprochen wird, klingt das nach 58 Anbietern, die um Marktanteile ringen. Die Wirklichkeit sieht anders aus, und sie ist erst seit April 2026 öffentlich.
Ein einziges Produkt trägt den Markt
Anteil an allen abgegebenen Freischaltcodes und an allen Ausgaben, Kalenderjahr 2025.
Auf ein einziges Produkt — das Adipositas-Programm Oviva Direkt — entfielen 2025 rund 44 Prozent aller abgegebenen Codes und knapp 40 Prozent aller Ausgaben. Nummer zwei, die Rücken-App Vivira, kommt auf 5,9 Prozent. Nummer drei, die Tinnitus-App Kalmeda, auf 4,5 Prozent. REPORTED
Das erklärt auch das viel zitierte Wachstum. Die Ausgaben stiegen 2025 um 63 Prozent — getragen im Wesentlichen von einer Anwendung, in einem Indikationsgebiet, das gerade aus ganz anderen Gründen im Fokus steht: Adipositas. Erstmals überholten Stoffwechselerkrankungen (34,2 Prozent) die psychischen Erkrankungen (28,3 Prozent) als größte Nutzungsgruppe.
Was das für alle anderen bedeutet
Rechnen wir die Folge aus. Von 171 Millionen Euro Jahresausgaben entfallen rund 68 Millionen auf Oviva Direkt. Bleiben etwa 103 Millionen Euro — für 54 andere Anwendungen. Im Durchschnitt sind das rund 1,9 Millionen Euro Umsatz je Anwendung und Jahr, brutto, vor Vertrieb, vor Studien, vor Zulassung, vor jeder Preisverhandlung.
Und Durchschnitt heißt hier: Die tatsächliche Verteilung ist noch schiefer, weil auch unter den 54 einige deutlich mehr abbekommen als andere.
Der Größenvergleich, der weh tut
Eine Praxissoftware-Firma wie CompuGroup Medical macht in einem Jahr mehr als das Siebenfache dessen, was die Krankenkassen im selben Jahr für sämtliche Apps auf Rezept zusammen ausgeben. Doctolib allein macht rund das Zweieinhalbfache. Die Software, die Ärzte beim Verwalten unterstützt, ist ein weit größeres Geschäft als die Software, die Patienten behandelt.
Warum sich das so schwer bewegen lässt
Wer eine App verordnen soll, sind Ärzte. 51 Prozent aller DiGA-Verordnungen stellen Hausärztinnen und Hausärzte aus — und davon entfallen 72 Prozent auf Stoffwechselerkrankungen. Bei einzelnen Produkten ist die Konzentration noch extremer: 86 Prozent der Oviva-Verordnungen kommen von Hausärzten, 78 Prozent der Vivira-Verordnungen von der Orthopädie, 86 Prozent der Kalmeda-Verordnungen von HNO-Ärzten. Was das heißt: Jede App hängt an einer einzigen Facharztgruppe. Wer diese Gruppe nicht erreicht, hat keinen Markt — und Ärzte erreicht man in Deutschland nicht über Werbung, sondern über Fachgesellschaften, Leitlinien und Praxissoftware. Das ist kein Wachstumsproblem, das man mit Marketingbudget löst.
Vier Befunde, die in fast jeder Berichterstattung über Apps auf Rezept fehlen — und die die Bewertung des Modells umdrehen.

Die übliche Sicht: „Die Hersteller nehmen die Kassen aus. Deshalb muss rückwirkend nachverhandelt werden.“ Oder, aus der Gegenrichtung: „Die Rückwirkung ist eine Enteignung, die Firmen zerstört.“
Der übersehene Mechanismus: Beide Sätze stimmen — gleichzeitig, in derselben Regel. Das Erprobungsjahr ist eine Einbahnstraße in beide Richtungen. Nach unten trägt der Hersteller das volle Preisrisiko: Was er im ersten Jahr eingenommen und ausgegeben hat, kann rückwirkend zurückgefordert werden, und zwar umso mehr, je erfolgreicher er war. Nach oben trägt die Kasse das volle Nutzenrisiko: Zahlt sie ein Jahr lang für eine App, die den Nutzen nie belegt, bekommt sie nichts zurück.
Beleg: Rückzahlungspflicht nach § 134 SGB V, dokumentiert im Fall zanadio (rückwirkend bis Oktober 2021, Insolvenz Mai 2023). Umgekehrt der Bericht des GKV-Spitzenverbands zu den 16 gestrichenen Apps: kein Rückzahlungsanspruch aus dem ersten Aufnahmejahr, Gesamtvolumen 7,53 Millionen Euro. Insgesamt beziffert der Bericht mehr als 25 Millionen Euro gefährdete Forderungen.
Warum das die Entscheidung ändert: Wer nur eine Seite sieht, schlägt eine Reform vor, die die andere Seite verschlimmert. Die 20-Prozent-Erfolgskomponente ab 2026 verschärft den Zahlungsdruck auf Hersteller, ohne das Rückforderungsrisiko der Kassen zu senken. Die eigentliche Reform wäre banal: ein gedeckelter Preis ab Tag eins statt einer Rückrechnung nach zwölf Monaten. Genau das haben Frankreich und die Schweiz gemacht.

Die übliche Sicht: „Der DiGA-Markt wächst kräftig — 63 Prozent im Jahr 2025. Das ist ein wachsender Markt für digitale Therapien.“
Der übersehene Mechanismus: Das Wachstum ist kein Kategorie-Effekt, sondern ein Produkt-Effekt. Eine einzige Adipositas-Anwendung steht für rund 44 Prozent der Abgaben und 40 Prozent der Ausgaben. Sie wächst in einem Indikationsgebiet, das gerade aus Gründen boomt, die mit Software nichts zu tun haben. Für alle übrigen 54 Anwendungen zusammen bleibt ein Markt von rund 103 Millionen Euro im Jahr.

Warum das die Entscheidung ändert: Die richtige Frage an einen Anbieter ist nicht „Wie schnell wächst der DiGA-Markt?“, sondern „Wie groß ist deine Indikation, und wie viele Ärzte einer einzigen Facharztgruppe musst du überzeugen?“ Wer diese Zahl nicht nennen kann, plant gegen einen Marktdurchschnitt, den es nicht gibt.

Die übliche Sicht: „Deutschland ist internationaler Vorreiter bei digitalen Gesundheitsanwendungen. Andere Länder ziehen nach.“
Der übersehene Mechanismus: Andere Länder ziehen tatsächlich nach — aber keines hat das deutsche Modell übernommen. Alle haben dieselbe Idee kopiert (schneller Zugang, Evidenz später) und dabei systematisch den teuersten deutschen Konstruktionsfehler vermieden: den frei gesetzten Herstellerpreis im ersten Jahr. Frankreich deckelt auf 780 Euro im Jahr, ab Tag eins. Die Schweiz berechnet den Preis nach einer offengelegten Kostenformel, ab Tag eins. Belgien zahlt erst auf der höchsten Prüfstufe. Großbritannien zahlt gar keinen Produktpreis. Dänemark empfiehlt, ohne zu zahlen.
Beleg: Frankreich: 435 Euro im ersten Quartal, danach 38,30 Euro monatlich, maximal 780 Euro im Jahr, ein Jahr, nicht verlängerbar. Schweiz: Höchstvergütungsbetrag aus einer Formel mit offengelegten Parametern, erstes Produkt seit 1. Juli 2026. Eine HTA-Vergleichsstudie hält für Deutschland fest, dass im Verfahren „no economic analysis is requested or performed“ und dass die Bewertungen nicht öffentlich sind.
Warum das die Entscheidung ändert: Deutschland ist Vorreiter im Sinne des Zeitpunkts und des Volumens, nicht der Methodik. Wer das deutsche Modell für ein anderes Land plant oder als Exportgut verkauft, verkauft ein Testlabor als Blaupause. Und wer als Anbieter auf europäische Skalierung setzt: Es gibt keine europäische Skalierung. Es gibt sechs verschiedene Systeme mit sechs verschiedenen Preislogiken.

Die übliche Sicht: „Digital Health ist unterfinanziert — es fließt zu wenig Kapital in den Sektor.“
Der übersehene Mechanismus: Für den erstatteten Teil gilt das Gegenteil. In deutsche Digital-Health-Unternehmen flossen 2025 je nach Zählweise 520 bis 610 Millionen US-Dollar Wagniskapital. Der gesamte erstattete Markt, den diese Unternehmen bedienen können, betrug im selben Jahr 171 Millionen Euro. Es fließt mehr Kapital in den Sektor, als der regulierte Markt in einem Jahr ausschüttet.
Beleg: Deutschland 2025 — 523,9 Millionen US-Dollar über 48 Deals nach dem einen Tracker, 612 Millionen US-Dollar nach dem anderen; die beiden widersprechen sich sogar in der Richtung (Anstieg gegen Rückgang). GKV-Ausgaben 2025: 171 Millionen Euro, davon rund 40 Prozent auf ein Produkt.
Warum das die Entscheidung ändert: Das Kapital sucht nicht den DiGA-Markt — es sucht etwas anderes und nimmt die DiGA-Zulassung mit. Die größten deutschen Runden 2025 und 2026 gingen an ein medizinisches Nachschlagewerk, an Praxis-Abrechnung, an Klinik-Software und an Spracherkennung für die Pflege. Wo Geld in Digital Health verdient wird, ist es Infrastruktur, nicht Therapie.
Die nützlichste Übung bei jedem Gesundheitsmarkt ist, einem einzelnen Euro zu folgen. Nehmen wir eine durchschnittliche Verordnung nach der Preisverhandlung: 227 Euro.

Zwei Dinge fallen auf. Erstens: Für die Verordnung selbst gibt es kein Honorar. Die frühere Zusatzpauschale entfiel Ende 2022; seither ist der Aufwand in die allgemeine Versichertenpauschale eingerechnet. Vergütet wird nur die spätere Verlaufskontrolle, mit 8,15 Euro.
Zweitens — und das ist die eigentliche Merkwürdigkeit: Diese 8,15 Euro gibt es je Anwendung einzeln. Für jede dauerhaft aufgenommene DiGA muss der Bewertungsausschuss erst eine eigene Abrechnungsziffer schaffen. Für die Fatigue-App Untire, im September 2025 dauerhaft aufgenommen, wurde keine geschaffen. Wer sie verordnet und begleitet, bekommt dafür nichts. CONFIRMED
Was das heißt: Die Person, die entscheidet, ob eine App zum Einsatz kommt, verdient daran fast nichts und trägt den gesamten Erklär- und Nachfrageaufwand. Das ist kein Anreizproblem am Rand — es ist der Grund, warum die Verordnungen sich auf wenige Produkte konzentrieren, für die es eingespielte Wege gibt.
Der Markt im Größenvergleich
Wo im deutschen Digital-Health-Markt tatsächlich Umsatz entsteht

Eine Warnung zu den Zahlen, die alle zitieren
AREF SIGNAL Die offiziellen Jahreswerte addieren sich nicht zur offiziellen Summe
Der DiGA-Bericht nennt für 2020/21 bis 2025 die Jahreswerte 20, 60, 64, 106 und 171 Millionen Euro. Zusammengerechnet sind das 421 Millionen. Als kumulierte Gesamtsumme nennt derselbe Bericht 401 Millionen. Die Differenz beträgt exakt 20 Millionen Euro.
Die Gegenprobe über den Vorjahresbericht (234 Millionen kumuliert bis Ende 2024) stützt die kleinere Zahl: 401 minus 171 ergibt 230, was zu 234 passt. Einer der Jahreswerte ist also zu hoch — welcher, ist aus den öffentlichen Unterlagen nicht ableitbar.
Praktische Folge: Wer mit diesen Zahlen arbeitet, sollte den kumulierten Wert verwenden und die Jahreswerte als Näherung kennzeichnen. Und man sollte wissen, dass eine viel zitierte Fachpublikation im April 2026 die kumulierten 234 Millionen als Jahresausgabe 2024 wiedergegeben hat — ein Faktor von mehr als zwei.
Der Satz „Deutschland ist Vorreiter“ steht in fast jedem Text über DiGA. Er stimmt — für eine Bedeutung von Vorreiter, die man aussprechen sollte.
Niemand hat das deutsche Modell übernommen
Erstattungswege für digitale Anwendungen im Vergleich, Stand September 2026.
Frankreich hat gedeckelt
Frankreich startete 2023 ein Verfahren namens PECAN, das der deutschen Erprobung ähnelt: früher Zugang, Nachweis später. Aber der Preis ist von Anfang an festgelegt — 435 Euro im ersten Quartal, danach 38,30 Euro im Monat, höchstens 780 Euro im Jahr. Und die Frist beträgt ein Jahr, nicht verlängerbar.
Das Ergebnis ist ernüchternd: Über drei Jahre lang wurde keine einzige therapeutische Anwendung tatsächlich über PECAN erstattet. Die erste — ein Lese-Trainingsprogramm für Kinder mit Legasthenie — ist seit dem 3. August 2026 erstattungsfähig. Ein deutscher Anbieter, HelloBetter, scheiterte 2024 mit seinem Antrag. CONFIRMED
Die Schweiz rechnet den Preis aus
Die Schweiz hat den vielleicht interessantesten Weg gewählt. Seit dem 1. Juli 2026 werden digitale Anwendungen zur Verhaltenstherapie bei Depressionen erstattet — und der Preis wird nicht verhandelt, sondern nach einer veröffentlichten Formel berechnet. Die Parameter stehen in einem amtlichen Dokument: Betriebskosten höchstens 200.000 Franken im Jahr, Wartung höchstens 60.000, Markteintritt pauschal 250.000, zwei Franken je Codezeile, Abschreibung über drei Jahre.
Das erste Produkt ist deprexis. Der Höchstvergütungsbetrag beträgt 193,32 Franken für 90 Tage. Man mag die Formel für schematisch halten — aber sie ist von Tag eins bekannt, für alle gleich, und niemand muss hinterher etwas zurückzahlen. CONFIRMED
Großbritannien zahlt keinen Produktpreis
Der NHS kauft digitale Therapien aus dem laufenden Budget, nach einer Empfehlung des Bewertungsinstituts NICE und einer dreijährigen Datensammlungsphase. Es gibt keine Positivliste mit Preisen. Es gibt auch keine zentrale Ausgabenstatistik — niemand in Großbritannien kann sagen, was digitale Therapien den NHS kosten. Das ist der Preis dieses Modells.
Die USA bezahlen die Ärztin, nicht die App
Seit Januar 2025 gibt es in der amerikanischen Medicare-Abrechnung drei neue Ziffern für digitale psychiatrische Behandlungsgeräte. Sie vergüten die ärztliche Betreuungszeit rund um die Anwendung — etwa 40 bis 52 Dollar je Monatsintervall. Für das Produkt selbst gibt es keinen bundesweiten Preis; jeder regionale Kostenträger setzt ihn selbst fest.
Wie brutal das für Anbieter ist, zeigt der bekannteste Fall der Branche. Pear Therapeutics ging 2021 mit einer Bewertung von rund 1,6 Milliarden Dollar an die Börse. 2023 wurde das Unternehmen zerschlagen und in einer Insolvenzauktion für 6,05 Millionen Dollar an vier Käufer verkauft — ein Wertverlust von über 99 Prozent. Der Grund war nicht fehlende Wirksamkeit, sondern fehlende Erstattung. Die neuen Abrechnungsziffern kamen zwei Jahre zu spät. REPORTED
Dänemark empfiehlt, ohne zu zahlen
Dänemark veröffentlichte im Juni 2025 als eines der ersten Länder eine amtliche Empfehlungsliste für Gesundheits-Apps. Inzwischen stehen sieben darauf. Die Liste sagt ausdrücklich: keine Erstattung, kein Tarif, keine Nutzungsgebühr. Ein Qualitätssiegel ohne Geld.
Die Bilanz in einem Satz
Deutschland hat 1,6 Millionen Verordnungen und 401 Millionen Euro Ausgaben. Frankreich hat eine erstattete Therapie-App. Belgien acht. Die Schweiz eine. Dänemark keine. Deutschland ist mit Abstand das Land mit dem größten realen Versorgungsvolumen — und zugleich dasjenige mit der geringsten Prüftiefe. Eine Vergleichsstudie hält fest, dass im deutschen Verfahren keine gesundheitsökonomische Analyse verlangt wird und dass die Bewertungen nicht öffentlich einsehbar sind. In Großbritannien und Frankreich ist beides Standard.
Das Regelwerk ist gerade in Bewegung wie seit 2020 nicht mehr. Wer heute plant, plant gegen fünf Fristen gleichzeitig.
Was gilt, was kommt, und wann

Die drei Fristen, auf die es ankommt
15. Oktober 2026 — die erste Meldung. Ab diesem Tag müssen Hersteller erstmals melden, wie ihre Anwendung tatsächlich benutzt wird: Nutzungsdauer, Nutzungshäufigkeit und Abbruchquoten. Damit endet die heute größte Informationslücke — bislang liegen die Nutzungsdaten bei den Herstellern, nicht bei den Zahlern. CONFIRMED
15. April 2028 — was die Patienten sagen. Zweite Stufe: strukturiert erhobene Rückmeldungen der Nutzenden.
15. April 2029 — krankheitsspezifische Ergebnisse. Dritte Stufe: indikationsbezogene Fragebögen, also die eigentliche Wirkungsmessung im Alltag.
Dazu kommt seit dem 1. Januar 2026 die Pflicht, mindestens 20 Prozent des Vergütungsbetrags erfolgsabhängig zu gestalten.
Warum die erste Frist die wichtigste ist
Bis Oktober 2026 kann jeder Anbieter behaupten, seine App werde gut genutzt. Danach steht es in einer Meldung. Wer heute eine Nutzungsquote von deutlich unter der Hälfte hat, hat noch etwa ein Jahr, bevor das aktenkundig wird.
Für Kassen und Politik ist es die erste belastbare Grundlage überhaupt: Ohne Abbruchquoten war die Debatte über den Nutzen bislang eine Debatte über Studien, die im Schnitt 321 Teilnehmende hatten. Danach ist sie eine Debatte über 695.000 tatsächliche Anwendungen im Jahr.
Und was drumherum passiert
Zwei europäische Regelwerke ändern die Rahmenbedingungen unabhängig vom Erstattungsrecht. Der Europäische Gesundheitsdatenraum gilt ab März 2027 allgemein, für Patientenaktensysteme ab März 2029. Die KI-Verordnung bringt ab August 2028 Hochrisikopflichten für Medizinprodukte mit KI-Anteil — die Frist wurde im Juli 2026 um ein Jahr nach hinten verschoben. Wer heute ein KI-Modul in eine zugelassene Anwendung baut, baut in eine Anforderung hinein, deren Detailregeln noch nicht final sind. REPORTED
Wenn ein Markt klein ist und die Regeln hart sind, entscheidet sich vieles daran, wie geduldig das Kapital ist. Deshalb lohnt der Blick auf Geld und Ausgänge.
Das Kapital ist zurück — aber nicht für alle
Wagniskapital in Digital Health, in Milliarden US-Dollar.
Eine Warnung vorweg: Diese Zahlen sind weicher, als sie aussehen
Es gibt keine amtliche Statistik für Wagniskapital in Digital Health. Was zitiert wird, kommt von privaten Datenanbietern, die unterschiedliche Dinge zählen. Für Deutschland 2025 nennt der eine 523,9 Millionen US-Dollar über 48 Finanzierungsrunden und spricht von einem deutlichen Anstieg. Der andere nennt 612 Millionen und spricht von einem Rückgang. Beide veröffentlichen ihre Abgrenzung nicht. REPORTED
Eine dritte Quelle, die Sektoren breiter fasst, kommt für „Health“ insgesamt auf 1.074 Millionen Euro im Jahr 2025 — aber die drei größten darin gezählten Runden waren ein Biotech-Unternehmen, eine orthopädische Klinikkette und ein Spracherkennungs-Anbieter für die Pflege. Nur der dritte ist Digital Health im engeren Sinn.
Praktische Folge: Jede Aussage der Form „der deutsche Digital-Health-Markt hat X eingesammelt“ sollte man nach der Definition fragen. Meist ist die Antwort: eine andere als die, die man erwartet.
Wohin das Geld tatsächlich geht
Die größten deutschen Runden der letzten zwei Jahre erzählen eine klare Geschichte. 240 Millionen Euro für ein medizinisches Nachschlagewerk für Ärzte. 52 Millionen für Praxisabrechnung. 50 Millionen für Spracherkennung in der Pflegedokumentation. 31 Millionen für Klinik-Software. 200 Millionen Euro für einen europäischen Ernährungstherapie-Anbieter, der über eine Million Patienten betreut hat.
Kein einziger dieser Beträge ging an ein reines DiGA-Geschäft. Die klassischen DiGA-Anbieter finanzieren sich seit 2023 in einer anderen Größenordnung: gut sechs Millionen Euro für einen Depressions-Anbieter, eine Aufstockung auf 15 Millionen Dollar für einen urologischen Anbieter, eine Wachstumsfinanzierung ohne genannten Betrag für einen Rücken-Anbieter. Für mehrere bekannte Namen ließ sich seit 2022 überhaupt keine Runde mehr finden. REPORTED

Zwei Muster. Erstens: Fast alle Ausgänge sind Verkäufe an strategische Käufer, und fast keiner nennt einen Preis — was bei erfolgreichen Verkäufen unüblich ist. Zweitens: Der einzige offengelegte, große Preis gehört zu einem Unternehmen, dessen Wert im amerikanischen Geschäft lag. Der Käufer will die Marke in den USA ersetzen, aber in Deutschland behalten — wegen der DiGA-Zulassungen.
Der Bericht der Krankenkassen zählt bis Ende 2025 insgesamt neun Herstellerinsolvenzen und beziffert die dadurch gefährdeten Rückforderungen auf über 25 Millionen Euro. REPORTED
Der Vergleich, der alles einordnet
Doctolib — ein Terminbuchungs- und Praxissoftware-Anbieter, kein Therapieanbieter — machte 2025 einen Umsatz von rund 422 Millionen Euro und wurde im April 2026 in einem Anteilsverkauf mit 3,6 Milliarden Euro bewertet, 38 Prozent unter der Bewertung von 2022. Selbst nach diesem Abschlag ist ein einziges Infrastrukturunternehmen mehr wert, als der gesamte erstattete DiGA-Markt in zwanzig Jahren ausschütten würde.
Wenn man aus alldem eine einzige Marktlücke ableiten müsste, dann diese — und sie ist unangenehm klein, unangenehm langweilig und ziemlich sicher real.
DER KEIL Nutzungsnachweis als Dienstleistung, nicht als App
Der Anlass: Am 15. Oktober 2026 müssen alle DiGA-Hersteller erstmals melden, wie ihre Anwendung genutzt wird — Dauer, Häufigkeit, Abbruchquoten. Zwei Jahre später kommen Patientenrückmeldungen dazu, ein Jahr darauf krankheitsspezifische Ergebnisse. Parallel müssen seit Januar 2026 mindestens 20 Prozent des Preises erfolgsabhängig sein.
Das Problem, das dadurch entsteht: Rund 58 gelistete Anwendungen brauchen ab sofort eine belastbare, prüffähige, laufende Messinfrastruktur — Ereigniserfassung, Kohortenauswertung, Abbruchdefinitionen, die vor einer Kasse standhalten, und eine Berichtsform, die zum Meldeschema passt. Kaum ein Anbieter mit 1,9 Millionen Euro Jahresumsatz kann das intern bauen. Und der Fehler ist teuer: Wer eine Abbruchquote falsch definiert, verhandelt seinen eigenen Preis kaputt.
Warum das kein Produkt für Verbraucher ist: Der Kunde ist der Hersteller, nicht der Patient. Die Zahl der Kunden ist zweistellig. Der Vertrag ist eine Jahresgebühr, kein Nutzungspreis. Das ist unglamourös — und genau deshalb hat es kein Wagniskapital finanziert.
Wie groß das ist: Ehrlich gerechnet klein. Bei 58 Anbietern und einer plausiblen Jahresgebühr im mittleren fünfstelligen Bereich reden wir über einen adressierbaren Markt von wenigen Millionen Euro im Jahr — in Deutschland. Das ist kein Wagniskapital-Fall. Es ist ein gutes, profitables Beratungs- und Softwaregeschäft für ein Team von fünf Leuten. Wer es als Wachstumsgeschichte verkauft, lügt sich in die eigene Finanzplanung.
Was dagegen spricht: Der Bewertungsausschuss und der GKV-Spitzenverband könnten das Meldeschema so weit standardisieren, dass es zur Checkbox in bestehender Analytiksoftware wird. Die großen Anbieter — die, denen 44 Prozent des Marktes gehören — bauen es intern. Und die Frist kann sich verschieben; im deutschen Gesundheitsrecht ist das eher Regel als Ausnahme.
Einschätzung: Machbar, real terminiert, ehrlich begrenzt. Ein Geschäft für jemanden, der Regulierung lesen kann und keine Einhorn-Ambitionen hat — nicht für jemanden, der „im DiGA-Markt“ sein will.
Das Modell steht politisch unter Druck, und der Druck kommt inzwischen von der Kostenseite. Die entscheidende Weiche liegt bei der Frage, ob Versicherte künftig etwas dazuzahlen.
Was schon auf dem Tisch liegt
Eine Regierungskommission zur Finanzstabilität der Krankenkassen hat 2026 eine ganze Reihe von Sparvorschlägen gemacht. Einer davon betrifft DiGA direkt: eine Selbstbeteiligung von 10 Prozent des Vergütungsbetrags, in einer Variante 10 Prozent plus 15 Euro. Bei 227 Euro wären das rund 23 beziehungsweise 38 Euro je Verordnung.
Im Gesetz gelandet ist das bisher nicht. Ein Stabilisierungsgesetz passierte im Juli 2026 ohne DiGA-Regelung. Der Vorschlag liegt also weiter da — und die Ausgabenkurve macht ihn wahrscheinlicher, nicht unwahrscheinlicher. REPORTED
Szenario A — Zuzahlung kommt wahrscheinlich
Was passiert: Versicherte zahlen 10 Prozent oder 10 Prozent plus 15 Euro. Folge: Die Aktivierungsquote fällt. Heute lösen 82 Prozent ihren Code ein — bei einer Zuzahlung von 23 bis 38 Euro dürfte diese Quote spürbar sinken, vor allem bei niedrigen Einkommen. Das trifft die Anbieter doppelt: weniger Einlösungen bedeuten weniger Umsatz und schlechtere Nutzungsdaten, die ab Oktober 2026 gemeldet werden müssen. Wer daran denkt: Anbieter sollten heute wissen, wie preiselastisch ihre Zielgruppe ist. Fast keiner weiß es.
Szenario B — Preisdeckel von Anfang an möglich
Was passiert: Der Gesetzgeber ersetzt die freie Preissetzung im ersten Jahr durch einen Höchstbetrag ab Tag eins — das französische oder schweizerische Modell. Folge: Die Rückwirkung verliert ihren Schrecken, weil es nichts mehr zurückzurechnen gibt. Die Anschubfinanzierung, die heute 544 Euro je Verordnung beträgt, fällt weg; damit wird der Markteintritt für kapitalschwache Anbieter deutlich schwerer. Für wen gut, für wen schlecht: Gut für etablierte Anbieter mit Bestand, schlecht für Neugründungen. Es wäre die sauberste Reform und zugleich die, die die Anbieterzahl weiter senkt.
Szenario C — es bleibt, wie es ist nicht auszuschließen
Was passiert: Die neuen Melde- und Erfolgspflichten wirken, die Preise sinken weiter durch Verhandlung, sonst ändert sich nichts. Folge: Die Bereinigung läuft über Attrition statt über Reform. Die Anträge, die 2021 noch bei 68 lagen und 2025 bei 23, sinken weiter. Der Markt konsolidiert sich auf eine Handvoll Produkte mit klarer Facharztanbindung.
In allen drei Szenarien gilt dasselbe: Die Zahl der Anbieter sinkt. Sie unterscheiden sich nur darin, ob das schnell und geordnet passiert oder langsam und durch Insolvenzen.
Woran man erkennt, dass eine Aussage über Apps auf Rezept nicht belastbar ist — egal ob sie von einem Anbieter, einer Kasse oder einer Zeitung kommt.
„Die GKV hat 234 Millionen Euro für DiGA ausgegeben“ — ohne Jahresangabe. Das ist der kumulierte Wert seit 2020, nicht ein Jahreswert. Mehrere Fachpublikationen haben ihn als Jahreswert 2024 wiedergegeben. Bei jeder DiGA-Zahl zuerst fragen: kumuliert oder Jahr?
Effektstärken ohne Angabe der Kontrollgruppe. Gegen eine Warteliste sieht fast alles gut aus. Die ehrliche Zahl ist die gegen eine aktive Vergleichsgruppe — und die ist meist um den Faktor fünf bis zwanzig kleiner.
„Studienbelegte Wirksamkeit“ als Qualitätssiegel. Alle 23 untersuchten DiGA-Zulassungsstudien wurden unabhängig als mit hohem Verzerrungsrisiko behaftet eingestuft. Ausnahmslos. Die Frage ist nicht, ob es eine Studie gibt, sondern wie groß sie war und wogegen sie verglichen hat.
Marktzahlen, die Infrastruktur und Therapie vermischen. Wenn ein „Digital-Health-Markt“ von mehreren Milliarden die Rede ist, stecken darin Praxissoftware, Terminbuchung, Klinik-IT und Biotech. Der erstattete Therapiemarkt beträgt 171 Millionen Euro im Jahr.
Wachstumsraten ohne Produktaufschlüsselung. Die 63 Prozent Wachstum in 2025 sind zu großen Teilen ein Adipositas-Programm. Wer daraus auf sein eigenes Indikationsgebiet schließt, rechnet mit fremdem Rückenwind.
„Deutschland ist Vorbild für Europa.“ Kein Land hat das deutsche Preismodell übernommen. Alle Nachfolger haben genau den Teil ausgelassen, der hier Insolvenzen verursacht hat.
Nutzungszahlen aus Herstellerhand ohne Abbruchdefinition. Bis Oktober 2026 gibt es keine verbindliche Definition, was ein Abbruch ist. Jede vorher genannte Quote ist eine Hausdefinition.
Ein Anbieter, der auf die Frage nach seiner Facharztgruppe keine Zahl nennt. Verordnungen kommen in dieser Branche zu 78 bis 86 Prozent aus einer einzigen Gruppe. Wer das nicht weiß, hat keinen Vertrieb, sondern eine Hoffnung.
Eine App auf Rezept kostet Sie nichts und kann helfen — insbesondere in der Wartezeit auf einen Therapieplatz, die in Deutschland oft Monate beträgt. Der wichtigste Hinweis: Lösen Sie den Code ein. 18 Prozent aller ausgegebenen Codes werden nie eingelöst; die Kasse hat trotzdem bezahlt. Und wenn die App nach zwei Wochen nichts bringt, ist das eine Information für Ihre Ärztin — kein persönliches Versagen. Ein Ersatz für eine Psychotherapie ist keine dieser Anwendungen, und keine behauptet das.
Wenn Sie verordnen
Zwei Dinge lohnen sich zu wissen. Erstens: Für die Verordnung selbst gibt es kein Honorar, für die Verlaufskontrolle 8,15 Euro einmal im Behandlungsfall — und nur, wenn für die betreffende Anwendung überhaupt eine Abrechnungsziffer existiert. Zweitens: Die Evidenzlage ist bei den meisten Anwendungen schwächer als die Werbung suggeriert, aber nicht null. Für die Überbrückung bis zum Therapieplatz ist die realistische Alternative meist nicht „bessere Behandlung“, sondern „keine Behandlung“. Das ist der Vergleich, der zählt.
Wenn Sie gründen oder investieren
Rechnen Sie das Geschäft mit dem Verhandlungspreis, nicht mit dem Listenpreis — also mit rund 227 statt 544 Euro. Rechnen Sie damit, dass die Differenz aus dem ersten Jahr rückwirkend zurückgefordert werden kann, und legen Sie sie zurück, statt sie in Wachstum zu stecken. Bestimmen Sie Ihre Facharztgruppe namentlich und zählen Sie, wie viele Personen das sind. Und stellen Sie sich die Frage, die dieser Bericht nahelegt: Ist Ihr Geschäft die Therapie — oder die Infrastruktur um sie herum? Das Geld ist bisher fast ausschließlich im zweiten Teil verdient worden.
Wenn Sie Politik oder Kasse sind
Die drei Eingriffe mit dem besten Verhältnis von Wirkung zu Aufwand: Erstens ein Höchstbetrag ab Tag eins statt einer Rückrechnung nach zwölf Monaten — das beseitigt die Insolvenzursache, ohne die Kassen schlechter zu stellen. Zweitens eine gesundheitsökonomische Prüfung im Aufnahmeverfahren; heute wird keine verlangt, in Frankreich und Großbritannien ist sie Standard. Drittens die Veröffentlichung der Bewertungen. Dass die Prüfberichte nicht öffentlich sind, ist der Punkt, an dem internationale Vergleichsstudien Deutschland am schärfsten kritisieren — und er ließe sich ohne Gesetzesänderung beheben.
Das deutsche Modell funktioniert als Versorgungsangebot und scheitert als Geschäftsmodell — und der Grund dafür ist eine einzige Regel, die niemand außerhalb Deutschlands übernommen hat.
Nach allem, was in diesem Bericht steht, halte ich Folgendes für richtig, mit der jeweiligen Sicherheit.
Erstens, mit hoher Sicherheit: Die Rückwirkung ist der Konstruktionsfehler, nicht die Preishöhe. Dass ein Preis von 544 auf 227 Euro fällt, ist harte, aber normale Regulierung. Dass er rückwirkend gilt und der Erfolgreichere mehr zurückzahlen muss, ist etwas anderes: eine Regel, die genau die Anbieter bestraft, die das System eigentlich will. Zwei dokumentierte Insolvenzen, davon eine beim meistverordneten Produkt des Landes, sind keine Zufallsereignisse. Und der Beleg dafür, dass es anders geht, steht in vier Nachbarländern.
Zweitens, mit hoher Sicherheit: Es gibt keinen DiGA-Markt. Es gibt ein Produkt und einen langen Schwanz. 44 Prozent der Abgaben und 40 Prozent der Ausgaben entfallen auf eine einzige Anwendung. Für alle anderen zusammen bleiben rund 103 Millionen Euro im Jahr — im Schnitt 1,9 Millionen je Anbieter, brutto. Jede Geschäftsplanung, die mit „der Markt wächst um 63 Prozent“ beginnt, plant mit fremdem Wachstum.
Drittens, mit mittlerer Sicherheit: Die Evidenzfrage ist offen — und sie wird zugunsten der Apps ausgehen, aber zu einem niedrigeren Preis. Die Studien sind klein, verzerrungsanfällig und gegen Wartelisten geführt. Gleichzeitig zeigt die größte Kostenstudie einen echten, wenn auch kleineren Unterschied als meist behauptet — rund 585 Euro je Patient und Jahr, nicht 1.021 —, und eine Modellierung aus gesellschaftlicher Perspektive kommt auf Kosten je gewonnenem Lebensjahr, die weit unter den üblichen Zahlungsschwellen liegen. Mein Schluss: Diese Anwendungen sind wahrscheinlich etwas wert — vermutlich deutlich weniger als 227 Euro und deutlich mehr als null. Die Schwelle von 20,34 Euro je Quartal halte ich für zu streng, weil sie nur das Kassenbudget kennt und den Arbeitsausfall ignoriert, den die Gesellschaft trägt.
Viertens, mit hoher Sicherheit: Wer in diesem Feld Geld verdienen will, sollte auf Infrastruktur setzen, nicht auf Therapie. Ein Terminbuchungsanbieter macht das Zweieinhalbfache des gesamten erstatteten Marktes. Ein Praxissoftware-Anbieter das Siebenfache. Die größten deutschen Finanzierungsrunden der letzten zwei Jahre gingen ausnahmslos an Werkzeuge für Behandelnde, nicht an Behandlungen für Patienten. Das ist kein vorübergehender Zustand, sondern folgt daraus, wie das System Preise setzt: Infrastruktur wird verhandelt, Therapie wird festgesetzt.
Fünftens, was ich nicht weiß und niemand weiß: Wie viele Menschen eine verordnete App tatsächlich zu Ende benutzen. Das ist die zentrale Zahl dieses ganzen Feldes, und sie existiert bis heute nicht öffentlich. Am 15. Oktober 2026 wird sie erstmals gemeldet. Ich würde jede Aussage über den Nutzen dieses Systems — meine eigene eingeschlossen — an dem Tag noch einmal prüfen.
Und eine Einordnung zum Schluss, die man in der Debatte selten hört: Deutschland hat als erstes Land der Welt einen Rechtsanspruch darauf geschaffen, dass eine geprüfte Software zulasten der Krankenversicherung verordnet werden kann. Das war mutig und es funktioniert — 1,6 Millionen Menschen haben eine solche Anwendung freigeschaltet, und 401 Millionen Euro sind dafür geflossen, was gemessen an einem Gesamthaushalt von über 300 Milliarden Euro im Jahr weniger als anderthalb Promille sind. Der Streit dreht sich um eine sehr kleine Summe mit einer sehr großen Signalwirkung. Das ist gut so — Testläufe sollen billig sein. Man sollte nur aufhören, so zu tun, als sei der Test schon bestanden.
Was dieser Bericht nicht leisten kann
Absolute Verordnungszahlen je einzelner Anwendung veröffentlicht der DiGA-Bericht nicht. Die Anteilswerte für Oviva Direkt, Vivira und Kalmeda stammen aus Fachpublikationen, die den Bericht auswerten; die daraus abgeleiteten Eurobeträge sind eigene Rechnungen.
Die Jahresausgaben 2020 bis 2025 addieren sich auf 421 Millionen Euro, während derselbe Bericht 401 Millionen als kumulierte Summe nennt. Welcher Jahreswert zu hoch ist, lässt sich öffentlich nicht klären.
Eine echte Abbruchquote innerhalb der 90 Tage existiert in keiner öffentlichen Quelle. Die 83 Prozent ohne Folgerezept sind keine Abbruchquote. Belastbare Zahlen entstehen erst mit der Meldepflicht ab 15. Oktober 2026.
Umsätze einzelner Anbieter ließen sich nicht belegen. Die meisten sind kleine Gesellschaften mit verkürzten Jahresabschlüssen ohne Umsatzausweis; die kursierenden Schätzungen sind Schätzungen.
Die Wagniskapitalzahlen stammen von privaten Datenanbietern mit nicht offengelegter Abgrenzung. Zwei von ihnen widersprechen sich für Deutschland 2025 nicht nur in der Höhe, sondern in der Richtung.
Die Aussage, dass in Frankreich keine Rückzahlungspflicht besteht, stützt sich auf eine anwaltliche Analyse von 2023, nicht auf den geprüften Gesetzestext; der Direktabruf des französischen Gesetzesportals war nicht möglich.
Die genauen US-Erstattungsbeträge für die neuen Abrechnungsziffern konnten nicht aus einer Primärquelle bestätigt werden; die genannten 40 bis 52 Dollar stammen aus zwei sich widersprechenden Sekundärquellen.
Der tagesaktuelle Stand des BfArM-Verzeichnisses ließ sich nicht abrufen. Alle Bestandszahlen beziehen sich auf den 31. Dezember 2025.

Imagine going to the doctor with back pain or depression. Instead of a pill or a referral you are handed a prescription for a smartphone app. You send it to your health insurer, receive an activation code, download the app and use it for 90 days. The insurer pays. You pay nothing.
That is exactly what has existed in Germany since 2019 — and, in this form, in no other country in the world. These apps are officially called digitale Gesundheitsanwendungen, or DiGA. Colloquially: “apps on prescription”.
The idea behind it is a good one. The average wait for a psychotherapy place in Germany is nearly five months. An app is available immediately. If it helps, it bridges a real gap.
This report asks a different question: why are fewer and fewer companies willing to build these apps? The numbers say so unambiguously. In 2021 the responsible federal agency received 68 applications. In 2025 it received 23.
How the system works
How an app gets onto a prescription — and how its price comes about
Two paths running in parallel: the route to the patient on the left, the route to the price on the right.
Two things run in parallel. On the left, the route to the patient: the app must first be an approved medical device, then the Federal Institute for Drugs and Medical Devices — the BfArM — reviews the application. If the review is positive, the app enters a public directory and may be prescribed.
On the right, the route to the price. That is the real subject of this report. In the first year the manufacturer sets the price itself. After that the insurers negotiate with it. If they cannot agree, an arbitration board decides. The price set that way applies retroactively — including to every prescription of the first year.
The terms that keep coming up
DiGA — an app a doctor can prescribe and the health insurer pays for. BfArM — the federal agency that decides on admission. GKV-Spitzenverband — the umbrella body of the statutory health insurers; it negotiates the price on behalf of all of them. Reimbursement amount — the negotiated price the insurer pays from year two onwards. Trial listing — a provisional admission for twelve months. The manufacturer already earns money and only has to demonstrate benefit afterwards. Positive care effect — the statutory proof that the app does something. Either medically measurable, or as an improvement in the treatment process. Activation code — what the patient receives from the insurer in order to unlock the app. Medical device / MDR — software that assists in diagnosis or therapy counts legally as a medical device and needs approval under the EU Medical Device Regulation. Randomised controlled trial (RCT) — the gold standard of efficacy evidence: participants are allocated at random to two groups; one receives the treatment, the other does not. Effect size (Cohen's d) — a measure of how large a treatment effect is. Rough rule of thumb: 0.2 small, 0.5 medium, 0.8 large.
Three questions, three different answers
This report answers three questions separately, because the evidence differs for each:
Should I rely on this as a patient or a doctor? Sometimes yes — see Part 6 and Part 16.
Should I build one of these as a founder? Almost never — see Part 5 and Part 13.
Should I put money into it as an investor? Only with a very close reading of a single contractual clause — see Part 5 and Part 14.
Before the numbers, a distinction on which most investment decisions in this field founder. “Digital health” is not a market but an umbrella term. Underneath it sit five businesses with entirely different economics.

What this means: Anyone who reads a market figure for “digital health” and infers the potential of a therapy app is comparing practice software with prescription apps. These are different worlds, and the difference is two orders of magnitude. Part 3 does the arithmetic.
Start with the number that puts everything else in place. The insurers are required to report annually what they spend on DiGA. For the whole period from 1 September 2020 to 31 December 2025 the report states: 1.6 million apps dispensed and around €401 million. For 2025 alone: 695,000 dispensings and €171 million.

That is the entire market for apps on prescription in Germany — after six years, with a statutory entitlement, in a health system that spends more than €300 billion a year.
For comparison: CompuGroup Medical sells software to doctors' practices and hospitals. No patient has ever heard of it. Revenue in 2025: €1.213 billion. Doctolib, the appointment platform, reached €422 million. Neither sells therapy; both sell workflow — and neither has ever applied for reimbursement.
What €227 buys elsewhere
What €227 buys elsewhere in the health system

A single number says little until you can place it. A DiGA costs the insurer €227 on average for 90 days. One session of guideline psychotherapy costs it €114.54. So one app is worth roughly two therapy sessions — and about one sixth of what a quarter of weekly therapy costs.
From the insurer's point of view that is not expensive. From the manufacturer's point of view that is the problem: out of those €227 must come development, a randomised trial, medical device approval and sales.
How the €171 million is distributed
Very unevenly. Across the whole period, 37 per cent of use falls on metabolic conditions and 27 per cent on mental health — while mental health accounts for 55 per cent of the catalogue. A handful of metabolic apps generates more volume than three dozen mental health apps.
Within the metabolic group the volume concentrates further on a single provider: Oviva accounted for 29 per cent of all prescriptions in 2024 and around 44 per cent in 2025. REPORTED
Divide the €171 million naively by 58 listed apps and you get an average of €2.9 million of revenue per app per year. Because of the concentration, the actual figure for almost every provider is far below that — how far below, Part 7 works out.OWN CALCULATION
AREF SIGNAL The market figures people quote are measuring something else
Market research houses put the German digital health market for 2026 at around US$11.9 billion. Another provider gives European “digital therapeutics” as US$2.58 billion for 2025. So one country is four times the size of a continent — because different things are being counted. The large figure includes telemedicine, hospital IT, connected devices and services.
None of these figures measures the DiGA market. Real revenue is €171 million — roughly one and a half per cent of the US$11.9 billion. OWN CALCULATION
Anyone using a digital health market figure as a proxy for the revenue potential of a therapy app overestimates it by a factor of seventy. That is not a nuance. That is the difference between a business model and a misunderstanding.
Before arguing about efficacy, a simpler question is worth asking: do people use the app at all? The insurers can see this, because every activation code is registered.
Of a hundred prescriptions, seventeen remain

Of a hundred codes issued, 82 are redeemed at all. Eighteen end up in a drawer. And of those who redeem, 83 per cent obtain no new prescription after the 90 days. REPORTED
Those 83 per cent are not a dropout rate. Someone who does not seek a repeat prescription may be cured, may have lost interest, or may simply be past the indication. The problem is that the insurers cannot tell the difference. The usage data sits with the manufacturers, not with the payers. A genuine dropout rate within the 90 days appears in no public source.
That is a structural gap, not a research gap — and precisely why, from October 2026, the law requires manufacturers to report duration of use, frequency of use and dropout rates for the first time.
The harder benchmark sits in the app store
For health apps outside the prescription system there is independent usage data. A study of 93 apps for anxiety, depression and wellbeing, each with at least 10,000 installations, found: a median of 4.0 per cent daily active users. After 15 days 3.9 per cent remained, after 30 days 3.3 per cent.
These figures come from the open market. A doctor's prescription improves matters considerably — the 82 per cent activation rate shows that. But they describe the gravity every health app works against. And they explain why two thirds of all health apps ever built are no longer offered today.
And what happens to the providers?
A market its own providers are leaving
New applications to the BfArM and the composition of the directory.
Of 74 apps ever admitted, 16 were removed again — one in five. The reason is almost always the same: benefit could not be demonstrated. Thirty-four apps made the jump from trial listing to permanent admission, one in five of those only with a restricted scope.
And the most telling number is on the application side: 68 applications in 2021, 23 in 2025. A drop of two thirds in a market that is formally growing. Providers are voting with their feet.
Here is the actual finding of this report. The German model has a design fault that is not theoretical. It has demonstrably destroyed companies — and not the worst ones.
What happens to the price once it is negotiated

The procedure is set out in law. In the first year the manufacturer sets the price freely. After that the insurers' association negotiates with it. If no agreement is reached within nine months, an arbitration board decides within three further months. By the end of 2025 there had been 40 such prices; roughly one in three was set by arbitration.
Across all apps the price falls from an average of €544 to €227 — a drop of 59 per cent. The largest single documented reduction went from €2,077.40 to €247.81.
That alone would be harsh but ordinary price regulation. The design fault is something else.
The price, once set, applies retroactively
The pricing trap, worked through

Let us work it through. A provider launches at the average price of €544 and is prescribed 1,000 times in the first year. It takes in €544,000 and builds its company with it: staff, sales, further development.
After twelve months the price is set at €227 — retroactively, from the first day of sale. The new price now applies to the 1,000 prescriptions already billed. The difference of €317 per prescription must be repaid: €317,000, for revenue that has long since been spent.
And now the perverse part: the more successful the provider was in year one, the larger the clawback. Success is punished.
Two insolvencies, the same cause
Newsenselab, with the migraine app M-sense, was the first case. Arbitration cut the price from €219.98 to €10 — a fall of 95 per cent. The app left the directory in April 2022; insolvency followed in July 2022.
aidhere, with the obesity app zanadio, filed for insolvency at Hamburg district court on 30 May 2023. Arbitration had cut the price from €499.80 to €218, retroactive to October 2021. The company said the “reimbursement amount reduced by more than half and the resulting repayment claims” were “economically unsustainable”. At that point zanadio was Germany's most-prescribed app, with more than 30,000 users. The app survived — another company bought it — the firm did not.
The zanadio case went to court. On 20 February 2025 the Berlin-Brandenburg regional social court held that the arbitration award was “essentially covered by the board's wide margin of discretion”. It overturned only the point on trial duration. Appeal to the Federal Social Court was permitted; the judgment is not final. CONFIRMED
And in the other direction the rule does not apply
What insurers paid for apps that never proved their benefit

Here it becomes asymmetric. If a manufacturer charged too much in year one, it must repay. If an app never proves its benefit and is therefore removed, the opposite happens. The insurers' report puts it unambiguously:
For expenditure from a DiGA's first year of listing, the statutory insurers have no repayment claim even where the DiGA has not demonstrated a positive care effect.

For the 16 removed apps, insurers paid €7.53 million in the respective first year. They get none of it back. In 15 of the 16 cases no negotiated price was ever reached — they disappeared first.
What this means: The trial year is a one-way street in both directions. The manufacturer bears the full price risk on the downside. The insurer bears the full benefit risk. Nobody gains from this construction — which is why criticism that sees only one side always falls short.
What the legislator has made of it
Since 1 January 2026 at least 20 per cent of the price must be outcome-dependent. Since 1 February 2026 there is a duty of continuous outcome measurement, staged in three steps: from October 2026 usage data including dropout rates, from April 2028 patients' own assessments, from April 2029 condition-specific questionnaires. Both address the evidence gap. Neither addresses the retroactivity.
Now the question that ought to come first: do these apps help people? The honest answer is — probably yes, a bit, and nobody knows exactly how much. That sounds unsatisfying. It is nonetheless more precise than anything manufacturers or critics claim in public.
What an effect size means
To compare effects, research uses a measure called effect size. You do not need to do the arithmetic, only to know a rule of thumb: 0.2 is small, 0.5 medium, 0.8 large. More vividly: at an effect size of 0.2, an average treated person is better off than 58 out of 100 untreated ones. At 0.8 it is 79 out of 100.
The decisive point, however, is not the number itself but what it was compared against.
How far the effect shrinks when the comparison group gets something

Compare an app with a waiting list — that is, with people who get nothing at all — and it looks good. Compare it with a group that receives something else, and the effects shrink to a fifth or a twentieth. For quality of life, 0.35 becomes 0.02: practically nothing.
And now the important sentence: every German DiGA admission study uses inactive control groups. They therefore measure the friendlier of the two numbers. CONFIRMED
How small the studies are

An independent review of all 23 admission studies covering 21 DiGA reached a harsh verdict in August 2025. Every study reported medium to large effects — but: “All studies were evaluated to have an overall high risk of bias.” Every single one. Particularly in outcome measurement (21 of 23 studies) and because of missing data (15 of 23).
There is also a pattern worth knowing. In the intervention groups an average of 21.7 per cent dropped out, in the control groups only 11.8 per cent. So people given an app drop out almost twice as often as people sitting on a waiting list — and those who drop out often go unmeasured. That flatters the results in a familiar direction.
The strongest DiGA effect ever measured (effect size 1.79) comes from the smallest study, with 56 participants, against a waiting list. That is not evidence of manipulation. It is what statistics predicts: small studies vary more, and results that vary upwards are more likely to be published.
The one study that asks about money
Almost every study asks: are people better off? Few ask: is it worth the money? For the depression app deprexis there is both.
The result, precisely — and why it is usually misquoted
In a randomised trial with 3,805 insured people at a large health insurer, total costs in the app group fell by €1,021 per patient within a year (down 32 per cent). That number appears in nearly every summary.
What gets left out: costs in the comparison group fell too — by €436 (down 13 per cent). The actual difference between the groups is therefore about €585, not €1,021. That difference was just barely statistically significant (p = 0.04).
And one more thing: the study also examined individual cost components — medication, hospital, sick pay. Not one of them differed significantly between the groups (p = 0.14 / 0.25 / 0.16). Only the total is robust, not the story about which component produces the saving. CONFIRMED
A second, more recent modelling study puts the question the other way round: below what price would such an app be not merely equivalent to standard care but superior — cheaper and more effective? The answer: below €20.34 per quarter. Today's average reimbursement amount is €227 per 90 days — about eleven times that.CONFIRMED
Fairness requires the counter-calculation, and it comes from a different condition. For digital applications in low back pain, a health-economic model found costs of around €5,486 per quality-adjusted life year — from a societal perspective, that is, including lost working time. That is far below what health systems are usually willing to pay. By that yardstick the application is cheap. CONFIRMED
Both calculations are serious and they still contradict each other. The reason is banal and important: one asks “does the insurer save money?”, the other asks “is the health gain worth its price — to society?”. Count lost working time and you reach a completely different result than if you look only at the insurance budget.
Germany officially answers neither question. The DiGA procedure requires no health-economic analysis at all. In France and the United Kingdom it is standard. CONFIRMED
The same app, three countries, three verdicts
How unstable the ground is shows in a single product. deprexis is permanently listed in Germany. In Switzerland it has been reimbursed since 1 July 2026. And the English NHS rated it in 2024 as not compliant with its assessment criteria, on the grounds that substantial content did not align with NICE protocols. CONFIRMED
The same product, the same evidence, three opposing decisions. Anyone claiming the evidence question is settled — in either direction — has not read it.
When people talk about “the DiGA market” it sounds like 58 providers competing for share. Reality looks different, and has only been public since April 2026.
A single product carries the market

One single product — the obesity programme Oviva Direkt — accounted for around 44 per cent of all dispensed codes and just under 40 per cent of all spending in 2025. Number two, the back-pain app Vivira, reaches 5.9 per cent. Number three, the tinnitus app Kalmeda, 4.5 per cent. REPORTED
That also explains the much-quoted growth. Spending rose 63 per cent in 2025 — carried essentially by one application, in an indication that happens to be in the spotlight for entirely unrelated reasons: obesity. For the first time metabolic conditions (34.2 per cent) overtook mental health (28.3 per cent) as the largest group of use.
What that means for everyone else
Work out the consequence. Of €171 million in annual spending, roughly €68 million goes to Oviva Direkt. That leaves about €103 million — for 54 other applications. On average that is around €1.9 million of revenue per application per year, gross, before sales, before trials, before approval, before any price negotiation.
And “average” here means: the actual distribution is more skewed still, because some of the 54 take considerably more than others.
The size comparison that stings
A practice-software company such as CompuGroup Medical turns over more than seven times, in one year, what the health insurers spend in the same year on every app on prescription combined. Doctolib alone turns over roughly two and a half times as much. The software that helps doctors with administration is a far bigger business than the software that treats patients.
Why this is so hard to shift
The people who have to prescribe are doctors. General practitioners write 51 per cent of all DiGA prescriptions — and 72 per cent of theirs go to metabolic conditions. For individual products the concentration is more extreme still: 86 per cent of Oviva prescriptions come from GPs, 78 per cent of Vivira prescriptions from orthopaedics, 86 per cent of Kalmeda prescriptions from ENT specialists. What this means: every app hangs on a single medical specialty. Miss that specialty and you have no market — and in Germany you do not reach doctors through advertising, but through professional societies, guidelines and practice software. That is not a growth problem you solve with a marketing budget.
Four findings absent from almost all reporting on apps on prescription — and which invert the assessment of the model.

The conventional view: “Manufacturers are fleecing the insurers. That is why prices must be renegotiated retroactively.” Or, from the other side: “The retroactivity is expropriation that destroys companies.”
The overlooked mechanism: Both sentences are true — simultaneously, within the same rule. The trial year is a one-way street in both directions. On the downside the manufacturer carries the full price risk: whatever it earned and spent in year one can be clawed back, and the more successful it was, the more. On the upside the insurer carries the full benefit risk: pay for a year for an app that never demonstrates benefit and you get nothing back.
Evidence: The repayment obligation under § 134 SGB V, documented in the zanadio case (retroactive to October 2021, insolvency May 2023). Conversely the insurers' report on the 16 removed apps: no repayment claim from the first year of listing, €7.53 million in total. The report puts more than €25 million of receivables at risk overall.
Why this changes the decision: Anyone who sees only one side proposes a reform that makes the other side worse. The 20 per cent outcome component from 2026 tightens the payment pressure on manufacturers without reducing the insurers' clawback risk. The real reform would be mundane: a capped price from day one instead of a recalculation after twelve months. Which is exactly what France and Switzerland did.

The conventional view: “The DiGA market is growing strongly — 63 per cent in 2025. This is a growing market for digital therapies.”
The overlooked mechanism: The growth is not a category effect but a product effect. One single obesity application accounts for about 44 per cent of dispensings and 40 per cent of spending. It is growing in an indication booming for reasons that have nothing to do with software. For all 54 remaining applications combined, a market of roughly €103 million a year is left.
Evidence: GKV-Spitzenverband DiGA report for 2025: Oviva Direkt 44.1 per cent of dispensings, 39.5 per cent of spending; metabolic overtakes mental health as the largest indication group (34.2 against 28.3 per cent). Annual spending 2025: €171 million.
Why this changes the decision: The right question to a provider is not “how fast is the DiGA market growing?” but “how large is your indication, and how many doctors of a single specialty must you convince?” Anyone who cannot state that number is planning against a market average that does not exist.

The conventional view: “Germany is the international pioneer in digital health applications. Other countries are following.”
The overlooked mechanism: Other countries are indeed following — but none has adopted the German model. All copied the same idea (fast access, evidence later) while systematically avoiding the most expensive German design fault: the freely set manufacturer price in year one. France caps at €780 a year, from day one. Switzerland calculates the price from a published cost formula, from day one. Belgium pays only at the top validation level. The United Kingdom pays no product price at all. Denmark endorses without paying.
Evidence: France: €435 for the first quarter, then €38.30 monthly, maximum €780 a year, one year, non-renewable. Switzerland: a maximum reimbursement amount from a formula with disclosed parameters, first product since 1 July 2026. A comparative HTA study records for Germany that “no economic analysis is requested or performed” and that the assessments are not publicly available.
Why this changes the decision: Germany is a pioneer in the sense of timing and volume, not of method. Anyone planning the German model for another country, or selling it as an export, is selling a test laboratory as a blueprint. And for providers betting on European scale: there is no European scale. There are six different systems with six different pricing logics.

The conventional view: “Digital health is underfunded — too little capital flows into the sector.”
The overlooked mechanism: For the reimbursed part the opposite holds. Depending on how you count, US$520–610 million of venture capital flowed into German digital health companies in 2025. The entire reimbursed market those companies can serve was worth €171 million that year. More capital flows into the sector than the regulated market pays out in a year.
Evidence: Germany 2025 — US$523.9 million across 48 deals according to one tracker, US$612 million according to another; the two disagree even on the direction (increase versus decline). Statutory insurer spending 2025: €171 million, roughly 40 per cent of it on one product.
Why this changes the decision: The capital is not looking for the DiGA market — it is looking for something else and taking the DiGA listing along. The largest German rounds of 2025 and 2026 went to a medical reference work, to practice billing, to hospital software and to speech recognition for nursing care. Where money is made in digital health, it is infrastructure, not therapy.
The most useful exercise in any health market is to follow a single euro. Take an average prescription after price negotiation: €227.

Two things stand out. First: there is no fee for the prescription itself. The former supplementary fee lapsed at the end of 2022; since then the effort is folded into the general capitation payment. Only the later follow-up is remunerated, at €8.15.
Second — and this is the real oddity: that €8.15 exists per application, individually. For every permanently admitted DiGA the valuation committee must first create its own billing code. For the fatigue app Untire, permanently admitted in September 2025, none was created. Anyone who prescribes and supervises it is paid nothing for doing so. CONFIRMED
What this means: The person who decides whether an app is used earns almost nothing from it and carries the entire burden of explaining and answering questions. This is not an incentive problem at the margin — it is the reason prescriptions concentrate on a few products for which established routes exist.
The market in proportion

The DiGA report gives annual values for 2020/21 to 2025 of €20, €60, €64, €106 and €171 million. Added together that is €421 million. As a cumulative total the same report states €401 million. The difference is exactly €20 million.
Cross-checking against the previous year's report (€234 million cumulative to end-2024) supports the smaller figure: 401 minus 171 gives 230, which fits 234. So one of the annual values is too high — which one cannot be derived from the public documents.
Practical consequence: anyone working with these numbers should use the cumulative value and mark the annual values as approximate. And it is worth knowing that a widely cited trade publication reported the cumulative €234 million as annual 2024 spending in April 2026 — a factor of more than two.
The sentence “Germany is the pioneer” appears in almost every text about DiGA. It is true — for a meaning of pioneer worth spelling out.
Nobody adopted the German model

France capped it
In 2023 France launched a scheme called PECAN, similar to the German trial listing: early access, evidence later. But the price is fixed from the outset — €435 for the first quarter, then €38.30 a month, at most €780 a year. And the term is one year, non-renewable.
The result is sobering: for more than three years not a single therapeutic application was actually reimbursed under PECAN. The first — a reading-training programme for children with dyslexia — became reimbursable on 3 August 2026. A German provider, HelloBetter, had its application rejected in 2024. CONFIRMED
Switzerland calculates the price
Switzerland has chosen perhaps the most interesting route. Since 1 July 2026 digital applications for cognitive behavioural therapy in depression are reimbursed — and the price is not negotiated but calculated from a published formula. The parameters are set out in an official document: operating costs at most CHF 200,000 a year, maintenance at most CHF 60,000, market entry a flat CHF 250,000, two francs per line of code, depreciation over three years.
The first product is deprexis. The maximum reimbursement amount is CHF 193.32 for 90 days. One may find the formula mechanical — but it is known from day one, identical for everyone, and nobody has to pay anything back afterwards. CONFIRMED
The United Kingdom pays no product price
The NHS buys digital therapies from its running budget, following a recommendation by the assessment body NICE and a three-year evidence-generation phase. There is no positive list with prices. There is also no central spending statistic — nobody in the United Kingdom can say what digital therapies cost the NHS. That is the price of this model.
The United States pays the doctor, not the app
Since January 2025 US Medicare billing has had three new codes for digital mental health treatment devices. They remunerate the physician's management time around the application — roughly $40 to $52 per monthly interval. For the product itself there is no national price; each regional contractor sets its own.
How brutal this is for providers is shown by the industry's best-known case. Pear Therapeutics went public in 2021 at a valuation of roughly $1.6 billion. In 2023 the company was broken up and sold in a bankruptcy auction for $6.05 million to four buyers — a loss of over 99 per cent. The cause was not missing efficacy but missing reimbursement. The new billing codes arrived two years too late. REPORTED
Denmark endorses without paying
In June 2025 Denmark became one of the first countries to publish an official endorsement list for health apps. Seven are now on it. The list says explicitly: no reimbursement, no tariff, no per-use fee. A quality mark without money.
The balance in one sentence
Germany has 1.6 million prescriptions and €401 million of spending. France has one reimbursed therapy app. Belgium has eight. Switzerland one. Denmark none. Germany is by far the country with the largest real volume of care — and simultaneously the one with the shallowest assessment. A comparative study records that the German procedure requires no health-economic analysis and that the assessments are not publicly available. In the United Kingdom and France both are standard.
The rulebook is in more motion than at any time since 2020. Anyone planning today is planning against five deadlines at once.
What applies, what is coming, and when

The three deadlines that matter
15 October 2026 — the first report. From that day manufacturers must report for the first time how their application is actually used: duration of use, frequency of use and dropout rates. That closes today's largest information gap — until now the usage data has sat with manufacturers, not payers. CONFIRMED
15 April 2028 — what patients say. Second stage: structured feedback from users.
15 April 2029 — condition-specific outcomes. Third stage: indication-related questionnaires, that is, actual measurement of effect in everyday care.
On top of that, since 1 January 2026 at least 20 per cent of the reimbursement amount must be outcome-dependent.
Why the first deadline is the most important
Until October 2026 any provider can claim its app is well used. After that it is in a report. Anyone whose usage rate is well below half today has roughly one more year before that becomes a matter of record.
For insurers and politicians it is the first solid basis of any kind: without dropout rates, the debate about benefit has so far been a debate about studies averaging 321 participants. Afterwards it becomes a debate about 695,000 actual uses a year.
And what is happening around it
Two European rulebooks change the framework independently of reimbursement law. The European Health Data Space applies generally from March 2027, and to patient record systems from March 2029. The AI Act brings high-risk obligations for medical devices with an AI component from August 2028 — the deadline was pushed back by a year in July 2026. Anyone building an AI module into an approved application today is building into a requirement whose detailed rules are not yet final. REPORTED
When a market is small and the rules are hard, much depends on how patient the capital is. Hence a look at money and exits.
Capital is back — but not for everyone
Venture capital in digital health, in billions of US dollars.
A warning first: these numbers are softer than they look
There is no official statistic for venture capital in digital health. What gets quoted comes from private data providers counting different things. For Germany in 2025 one gives US$523.9 million across 48 financing rounds and describes a clear increase. The other gives US$612 million and describes a decline. Neither publishes its definition. REPORTED
A third source, defining sectors more broadly, arrives at €1,074 million for “health” overall in 2025 — but the three largest rounds counted in it were a biotech company, an orthopaedic clinic chain and a speech-recognition provider for nursing care. Only the third is digital health in the narrow sense.
Practical consequence: any statement of the form “the German digital health market raised X” should be met with a question about the definition. Usually the answer is: a different one from the one you expect.
Where the money actually goes
The largest German rounds of the past two years tell a clear story. €240 million for a medical reference work for doctors. €52 million for practice billing. €50 million for speech recognition in nursing documentation. €31 million for hospital software. €200 million for a European nutritional therapy provider that has treated over a million patients.
Not one of those sums went to a pure DiGA business. The classic DiGA providers have been financing themselves on a different scale since 2023: just over €6 million for a depression provider, a top-up to US$15 million for a urology provider, a growth financing of undisclosed size for a back-pain provider. For several well-known names no round at all could be found since 2022. REPORTED

Two patterns. First: almost every exit is a sale to a strategic buyer, and almost none names a price — unusual for successful sales. Second: the only disclosed large price belongs to a company whose value lay in its American business. The buyer intends to replace the brand in the United States but keep it in Germany — because of the DiGA listings.
The insurers' report counts nine manufacturer insolvencies to the end of 2025 and puts the receivables thereby at risk at more than €25 million. REPORTED
The comparison that settles it
Doctolib — an appointment-booking and practice-software provider, not a therapy provider — had revenue of about €422 million in 2025 and was valued at €3.6 billion in an April 2026 share sale, 38 per cent below its 2022 valuation. Even after that markdown, one single infrastructure company is worth more than the entire reimbursed DiGA market would pay out in twenty years.
If one had to derive a single market gap from all of this, it would be this one — and it is uncomfortably small, uncomfortably boring and fairly certainly real.
THE WEDGE Usage evidence as a service, not as an app
The trigger: On 15 October 2026 every DiGA manufacturer must report for the first time how its application is used — duration, frequency, dropout rates. Two years later patient feedback is added, a year after that condition-specific outcomes. In parallel, since January 2026 at least 20 per cent of the price must be outcome-dependent.
The problem this creates: Around 58 listed applications now need robust, auditable, continuous measurement infrastructure — event capture, cohort analysis, dropout definitions that will survive scrutiny by an insurer, and a reporting format matching the statutory schema. Hardly any provider on €1.9 million of annual revenue can build that in-house. And the mistake is expensive: define a dropout rate wrongly and you negotiate your own price down.
Why this is not a consumer product: The customer is the manufacturer, not the patient. The number of customers is in the dozens. The contract is an annual fee, not a usage price. That is unglamorous — and precisely why no venture capital has funded it.
How big it is: Honestly, small. With 58 providers and a plausible annual fee in the mid five figures, we are talking about an addressable market of a few million euros a year — in Germany. This is not a venture case. It is a good, profitable consulting-and-software business for a team of five. Anyone selling it as a growth story is lying to their own financial plan.
What argues against it: The valuation committee and the insurers' association could standardise the reporting schema so far that it becomes a checkbox in existing analytics software. The large providers — the ones owning 44 per cent of the market — will build it in-house. And the deadline may slip; in German health law that is the rule rather than the exception.
Assessment: Feasible, genuinely dated, honestly bounded. A business for someone who can read regulation and has no unicorn ambitions — not for someone who wants to “be in the DiGA market”.
The model is under political pressure, and the pressure now comes from the cost side. The decisive switch is whether insured people will have to contribute.
What is already on the table
A government commission on the financial stability of the health insurers made a series of savings proposals in 2026. One concerns DiGA directly: a co-payment of 10 per cent of the reimbursement amount, in one variant 10 per cent plus €15. On €227 that would be roughly €23 or €38 per prescription.
It has not made it into law so far. A stabilisation act passed in July 2026 without a DiGA provision. So the proposal is still lying there — and the spending curve makes it more likely, not less. REPORTED
Scenario A — co-payment arrives likely
What happens: Insured people pay 10 per cent, or 10 per cent plus €15. Consequence: The activation rate falls. Today 82 per cent redeem their code — with a co-payment of €23 to €38 that rate should drop noticeably, above all among lower incomes. That hits providers twice: fewer redemptions mean less revenue and worse usage data, which must be reported from October 2026. Who should be thinking about this: providers should know today how price-elastic their target group is. Almost none does.
Scenario B — a price cap from the outset possible
What happens: The legislator replaces free pricing in year one with a maximum amount from day one — the French or Swiss model. Consequence: Retroactivity loses its sting, because there is nothing left to recalculate. The start-up financing that today amounts to €544 per prescription disappears; market entry becomes considerably harder for providers without capital. Good for whom, bad for whom: good for established providers with an installed base, bad for new entrants. It would be the cleanest reform and also the one that reduces the number of providers further.
Scenario C — nothing changes cannot be ruled out
What happens: The new reporting and outcome duties take effect, prices keep falling through negotiation, nothing else changes. Consequence: Consolidation runs through attrition rather than reform. Applications, 68 in 2021 and 23 in 2025, fall further. The market consolidates around a handful of products with clear specialty anchoring.
All three scenarios share one thing: the number of providers falls. They differ only in whether that happens quickly and in an orderly way, or slowly and through insolvencies.
How to recognise that a statement about apps on prescription is not sound — whether it comes from a provider, an insurer or a newspaper.
“Statutory insurers spent €234 million on DiGA” — with no year given. That is the cumulative figure since 2020, not an annual value. Several trade publications have reported it as annual 2024 spending. With every DiGA number, ask first: cumulative or annual?
Effect sizes without a stated control group. Against a waiting list, almost everything looks good. The honest figure is the one against an active comparison group — and that is usually five to twenty times smaller.
“Efficacy proven by studies” as a quality mark. All 23 DiGA admission studies examined were independently rated as carrying a high risk of bias. Without exception. The question is not whether a study exists, but how large it was and what it compared against.
Market figures that mix infrastructure with therapy. When a “digital health market” of several billion is mentioned, it contains practice software, appointment booking, hospital IT and biotech. The reimbursed therapy market is €171 million a year.
Growth rates without a product breakdown. The 63 per cent growth in 2025 is largely one obesity programme. Anyone extrapolating from that to their own indication is counting on somebody else's tailwind.
“Germany is the model for Europe.” No country has adopted the German pricing model. Every follower left out precisely the part that caused insolvencies here.
Usage figures from manufacturers without a dropout definition. Until October 2026 there is no binding definition of what a dropout is. Every rate quoted before then is a house definition.
A provider who cannot give a number when asked about its medical specialty. In this industry 78 to 86 per cent of prescriptions come from a single specialty. Anyone who does not know that has not a sales operation but a hope.
An app on prescription costs you nothing and can help — particularly during the wait for a therapy place, which in Germany often runs to months. The most important point: redeem the code. Eighteen per cent of all issued codes are never redeemed; the insurer has paid anyway. And if the app does nothing after two weeks, that is information for your doctor — not a personal failure. None of these applications is a substitute for psychotherapy, and none claims to be.
If you prescribe
Two things are worth knowing. First: there is no fee for the prescription itself, and €8.15 for follow-up, once per treatment case — and only if a billing code exists for that application at all. Second: for most applications the evidence is weaker than the marketing suggests, but it is not zero. For bridging the wait to a therapy place, the realistic alternative is usually not “better treatment” but “no treatment”. That is the comparison that counts.
If you are founding or investing
Model the business on the negotiated price, not the list price — €227, not €544. Assume the first-year difference can be clawed back retroactively, and set it aside instead of spending it on growth. Name your medical specialty explicitly and count how many people that is. And ask yourself the question this report points to: is your business the therapy — or the infrastructure around it? The money so far has been made almost entirely in the second.
If you are a policymaker or an insurer
The three interventions with the best ratio of effect to effort: first, a maximum amount from day one instead of a recalculation after twelve months — that removes the cause of insolvency without leaving insurers worse off. Second, a health-economic assessment in the admission procedure; none is required today, while in France and the United Kingdom it is standard. Third, publication of the assessments. That the review reports are not public is the point on which international comparative studies criticise Germany most sharply — and it could be fixed without changing a law.
The German model works as a care offering and fails as a business model — and the reason is a single rule that nobody outside Germany has adopted.
Given everything in this report, I hold the following, each with its own confidence.
First, with high confidence: the retroactivity is the design fault, not the price level. That a price falls from €544 to €227 is harsh but ordinary regulation. That it applies retroactively, and that the more successful party repays more, is something else: a rule that punishes precisely the providers the system says it wants. Two documented insolvencies, one of them at the country's most-prescribed product, are not chance events. And the proof that it can be done differently sits in four neighbouring countries.
Second, with high confidence: there is no DiGA market. There is one product and a long tail. Forty-four per cent of dispensings and 40 per cent of spending go to a single application. For everyone else roughly €103 million a year remains — an average of €1.9 million per provider, gross. Any business plan that begins “the market is growing 63 per cent” is planning on somebody else's growth.
Third, with medium confidence: the evidence question is open — and it will resolve in favour of the apps, but at a lower price. The studies are small, bias-prone and run against waiting lists. At the same time the largest cost study shows a real, if smaller, difference than usually claimed — about €585 per patient per year, not €1,021 — and a model from a societal perspective arrives at a cost per life year gained far below customary thresholds. My conclusion: these applications are probably worth something — probably considerably less than €227 and considerably more than zero. I regard the €20.34 per quarter threshold as too strict, because it knows only the insurance budget and ignores the lost working time society bears.
Fourth, with high confidence: anyone who wants to make money in this field should bet on infrastructure, not therapy. An appointment-booking provider turns over two and a half times the entire reimbursed market. A practice-software vendor seven times. The largest German financing rounds of the past two years went without exception to tools for practitioners, not treatments for patients. That is not a temporary state but a consequence of how the system sets prices: infrastructure is negotiated, therapy is imposed.
Fifth, what I do not know and nobody knows: how many people actually finish a prescribed app. That is the central number of this entire field, and to this day it does not exist publicly. On 15 October 2026 it will be reported for the first time. I would re-examine every statement about the benefit of this system — my own included — on that day.
And one closing observation rarely heard in the debate: Germany was the first country in the world to create a legal entitlement to have vetted software prescribed at the expense of health insurance. That was bold and it works — 1.6 million people have activated such an application, and €401 million has flowed for it, which against a total budget of more than €300 billion a year is less than one and a half parts per thousand. The argument is about a very small sum with a very large signalling effect. That is as it should be — pilot schemes ought to be cheap. One should simply stop pretending the pilot has already passed.
What this report cannot do
The DiGA report does not publish absolute prescription counts per application. The share figures for Oviva Direkt, Vivira and Kalmeda come from trade publications analysing the report; the euro amounts derived from them are own calculations.
The annual spending values for 2020 to 2025 sum to €421 million, while the same report states €401 million cumulative. Which annual value is too high cannot be resolved from public documents.
A genuine dropout rate within the 90 days exists in no public source. The 83 per cent without a repeat prescription are not a dropout rate. Solid figures will only arise with the reporting duty from 15 October 2026.
Revenues of individual providers could not be documented. Most are small companies filing abbreviated accounts without a revenue line; the estimates in circulation are estimates.
The venture capital figures come from private data providers with undisclosed definitions. Two of them contradict each other for Germany in 2025 not only on the level but on the direction.
The statement that France has no repayment obligation rests on a 2023 law-firm analysis, not on the verified statutory text; direct access to the French legislation portal was not possible.
The exact US reimbursement amounts for the new billing codes could not be confirmed from a primary source; the $40 to $52 quoted come from two contradictory secondary sources.

ایده پشت آن خوب است. میانگین انتظار برای یک جای درمان روانشناختی در آلمان نزدیک پنج ماه است. اپلیکیشن بیدرنگ در دسترس است. اگر کمک کند، شکاف واقعی را پر میکند.
پرسش این گزارش چیز دیگری است: چرا شمار شرکتهایی که میخواهند چنین اپلیکیشنی بسازند مدام کمتر میشود؟ اعداد این را روشن میگویند. در سال ۲۰۲۱ نهاد فدرال مربوطه ۶۸ درخواست دریافت کرد. در سال ۲۰۲۵ تنها ۲۳ درخواست.
نظام چگونه کار میکند
چگونه یک اپلیکیشن روی نسخه میآید — و قیمتش چگونه شکل میگیرد
دو مسیر موازی: مسیر رسیدن به بیمار در یک سو، مسیر تعیین قیمت در سوی دیگر.
دو چیز بهموازات هم پیش میرود. مسیر رسیدن به بیمار: اپلیکیشن نخست باید یک تجهیز پزشکی تأییدشده باشد، سپس مؤسسه فدرال داروها و تجهیزات پزشکی — بهاختصار BfArM — درخواست را بررسی میکند. اگر بررسی مثبت باشد، اپلیکیشن وارد فهرستی عمومی میشود و میتوان آن را تجویز کرد.
و مسیر دیگر، مسیر تعیین قیمت. موضوع اصلی این گزارش همین است. در سال اول سازنده خودش قیمت را تعیین میکند. پس از آن بیمههای درمانی با او مذاکره میکنند. اگر به توافق نرسند، یک هیئت داوری تصمیم میگیرد. قیمتی که اینگونه تعیین شود عطف به ماسبق میشود — یعنی برای همه تجویزهای سال اول هم اعمال میشود.
اصطلاحهایی که مدام تکرار میشوند
DiGA — اپلیکیشنی که پزشک میتواند تجویز کند و بیمه هزینهاش را میپردازد. با عددی شروع کنیم که بقیه را سر جای خود مینشاند. بیمههای درمانی موظفاند سالانه گزارش کنند چقدر برای DiGA هزینه کردهاند. برای کل دوره ۱ سپتامبر ۲۰۲۰ تا ۳۱ دسامبر ۲۰۲۵ آمده است: ۱٫۶ میلیون اپلیکیشن تحویلشده و حدود ۴۰۱ میلیون یورو. تنها برای سال ۲۰۲۵: ۶۹۵٬۰۰۰ تحویل و ۱۷۱ میلیون یورو.

برای مقایسه: CompuGroup Medical نرمافزار به مطبها و بیمارستانها میفروشد. هیچ بیماری نامش را نشنیده است. درآمد ۲۰۲۵: ۱٫۲۱۳ میلیارد یورو. Doctolib، سکوی نوبتدهی، به ۴۲۲ میلیون رسید. هیچکدام درمان نمیفروشند، بلکه گردش کار میفروشند — و هیچکدام هرگز درخواست بازپرداخت ندادهاند.

از نگاه بیمه این گران نیست. از نگاه سازنده مشکل همین است: از این ۲۲۷ یورو باید توسعه، یک کارآزمایی تصادفیشده، تأییدیه تجهیز پزشکی و فروش تأمین شود.
۱۷۱ میلیون چگونه توزیع میشود
بسیار نابرابر. در کل دوره، ۳۷ درصد استفاده به بیماریهای متابولیک و ۲۷ درصد به بیماریهای روانی میرسد — در حالی که بیماریهای روانی ۵۵ درصد فهرست را میسازند. مشتی اپلیکیشن متابولیک حجمی بیشتر از سه دوجین اپلیکیشن روان تولید میکند.
درون گروه متابولیک، حجم باز هم روی یک ارائهدهنده متمرکز میشود: Oviva در ۲۰۲۴ به ۲۹ درصد همه تجویزها رسید و در ۲۰۲۵ به حدود ۴۴ درصد. REPORTED

پیش از بحث بر سر اثربخشی، پرسش سادهتری ارزش دارد: آیا مردم اصلاً از اپلیکیشن استفاده میکنند؟ بیمهها این را میبینند، چون هر کد فعالسازی ثبت میشود.
از هر صد نسخه، هفده باقی میماند
سهم کدهای فعالسازی صادرشده که به مرحله بعدی میرسد.
از هر صد کد صادرشده، ۸۲ مورد اصلاً استفاده میشود. ۱۸ مورد در کشو میماند. و از میان کسانی که استفاده میکنند، ۸۳ درصد پس از ۹۰ روز نسخه تازهای نمیگیرند. REPORTED
این ۸۳ درصد نرخ رهاکردن نیست. کسی که نسخه بعدی نمیگیرد ممکن است درمان شده باشد، علاقهاش را از دست داده باشد، یا اصلاً از آن وضعیت عبور کرده باشد. مشکل این است که بیمهها تفاوت را نمیبینند. دادههای استفاده نزد سازندگان است، نه نزد پرداختکنندگان. نرخ واقعی رهاکردن در همان ۹۰ روز در هیچ منبع عمومی وجود ندارد.
این شکافی ساختاری است، نه شکاف پژوهشی — و دقیقاً به همین دلیل قانونگذار از اکتبر ۲۰۲۶ برای نخستین بار از سازندگان میخواهد مدت استفاده، تعداد دفعات استفاده و نرخ رهاکردن را گزارش کنند.
رقم مقایسهای سختتر در فروشگاه اپلیکیشن است
برای اپلیکیشنهای سلامت بیرون از نظام نسخه، دادههای مستقل استفاده وجود دارد. بررسی ۹۳ اپلیکیشن برای اضطراب، افسردگی و بهزیستی، هرکدام با دستکم ۱۰٬۰۰۰ نصب، نشان داد: بهطور میانه ۴٫۰ درصد کاربر فعال روزانه. پس از ۱۵ روز ۳٫۹ درصد ماندند و پس از ۳۰ روز ۳٫۳ درصد.
این ارقام از بازار آزاد میآید. نسخه پزشک وضع را بهروشنی بهتر میکند — ۸۲ درصد فعالسازی همین را نشان میدهد. اما این ارقام نیروی جاذبهای را توصیف میکنند که هر اپلیکیشن سلامت با آن دستوپنجه نرم میکند. و توضیح میدهند چرا دو سوم همه اپلیکیشنهای سلامتی که تا کنون ساخته شدهاند امروز دیگر عرضه نمیشوند.
و بر سر ارائهدهندگان چه میآید؟
بازاری که ارائهدهندگانش از آن بیرون میروند
درخواستهای تازه به BfArM و ترکیب فهرست.
از ۷۴ اپلیکیشن پذیرفتهشده، ۱۶ مورد دوباره حذف شد — یعنی از هر پنج، یکی. دلیل تقریباً همیشه یکی است: سودمندی اثبات نشد. ۳۴ اپلیکیشن از دوره آزمون به پذیرش دائمی رسیدند، و از هر پنج مورد یکی تنها با دامنه خدمات محدود.
و گویاترین عدد در سمت درخواستهاست: ۶۸ درخواست در سال ۲۰۲۱، ۲۳ درخواست در سال ۲۰۲۵. افت دو سوم در بازاری که رسماً رشد میکند. ارائهدهندگان با پای خود رأی میدهند.
یافته اصلی این گزارش اینجاست. مدل آلمانی یک ایراد ساختاری دارد که نظری نیست. این ایراد بهطور مستند شرکتها را نابود کرده است — و نه بدترینها را.
پس از آغاز مذاکره چه بر سر قیمت میآید
قیمت سازنده در سال اول در برابر مبلغ توافقشده، به یورو برای هر تجویز.
روال در قانون آمده است. در سال اول سازنده قیمت را آزادانه تعیین میکند. سپس اتحادیه بیمههای درمانی با او مذاکره میکند. اگر ظرف نُه ماه توافقی حاصل نشود، هیئت داوری ظرف سه ماه دیگر تصمیم میگیرد. تا پایان ۲۰۲۵ چهل قیمت از این دست وجود داشت؛ تقریباً از هر سه مورد یکی را هیئت داوری تعیین کرد.
در مجموع اپلیکیشنها قیمت از میانگین ۵۴۴ یورو به ۲۲۷ یورو میافتد — کاهش ۵۹ درصدی. بزرگترین کاهش تکموردیِ مستند از ۲٬۰۷۷٫۴۰ یورو به ۲۴۷٫۸۱ یورو بود.
این بهتنهایی تنظیمگری سختگیرانه اما عادی میبود. ایراد ساختاری چیز دیگری است.
قیمت تعیینشده عطف به ماسبق میشود
تله قیمتگذاری، با یک محاسبه کامل
نمونهای برای ارائهدهندهای با ۱۰۰۰ تجویز در سال اول.
بیایید حساب کنیم. ارائهدهندهای با قیمت میانگین ۵۴۴ یورو شروع میکند و در سال اول ۱۰۰۰ بار تجویز میشود. ۵۴۴٬۰۰۰ یورو دریافت میکند و با آن شرکتش را میسازد: کارکنان، فروش، توسعه.
پس از دوازده ماه قیمت روی ۲۲۷ یورو تعیین میشود — و آن هم عطف به ماسبق از نخستین روز فروش. برای همان ۱۰۰۰ تجویزی که پیشتر صورتحساب شده، حالا قیمت تازه اعمال میشود. تفاوت ۳۱۷ یورو برای هر تجویز باید بازگردانده شود: ۳۱۷٬۰۰۰ یورو، برای درآمدی که مدتهاست خرج شده است.
و حالا بخش وارونه ماجرا: هرچه ارائهدهنده در سال اول موفقتر بوده، بازپسگیری بزرگتر است. موفقیت مجازات میشود.
دو ورشکستگی، یک علت
Newsenselab با اپلیکیشن میگرن M-sense نخستین مورد بود. هیئت داوری قیمت را از ۲۱۹٫۹۸ یورو به ۱۰ یورو کاهش داد — افت ۹۵ درصدی. اپلیکیشن در آوریل ۲۰۲۲ از فهرست خارج شد و در ژوئیه ۲۰۲۲ ورشکستگی اعلام شد.
aidhere با اپلیکیشن چاقی zanadio در ۳۰ مه ۲۰۲۳ در دادگاه هامبورگ اعلام ورشکستگی کرد. هیئت داوری قیمت را از ۴۹۹٫۸۰ یورو به ۲۱۸ یورو کاهش داده بود، عطف به ماسبق تا اکتبر ۲۰۲۱. شرکت اعلام کرد «مبلغ پرداختِ بیش از نصف کاهشیافته و ادعاهای بازپرداختِ ناشی از آن» «از نظر اقتصادی تحملناپذیر» است. zanadio در آن زمان پرتجویزترین اپلیکیشن آلمان با بیش از ۳۰٬۰۰۰ کاربر بود. اپلیکیشن زنده ماند — شرکتی دیگر آن را خرید — اما خود شرکت نه.
پرونده zanadio به دادگاه رفت. دادگاه اجتماعی ایالتی برلین-براندنبورگ در ۲۰ فوریه ۲۰۲۵ رأی داد که حکم داوری «در اساس در چارچوب اختیار گسترده هیئت داوری» بوده است. تنها در مورد مدت دوره آزمون آن را نقض کرد. امکان فرجامخواهی به دادگاه فدرال اجتماعی داده شد؛ رأی قطعی نیست. CONFIRMED
و در جهت معکوس این قاعده برقرار نیست
بیمهها برای اپلیکیشنهایی که سودمندیشان اثبات نشد چه پرداختند
هزینه سال نخست ثبت هر اپلیکیشن، به میلیون یورو.
اینجا ماجرا نامتقارن میشود. اگر سازنده در سال اول زیادی گرفته باشد، باید بازگرداند. اگر اپلیکیشنی هرگز سودمندی را اثبات نکند و به همین دلیل حذف شود، عکس آن رخ میدهد. گزارش بیمههای درمانی آن را بیابهام مینویسد:
برای هزینههای سال نخست ثبت یک DiGA، بیمههای قانونی حق بازپرداخت ندارند، حتی اگر آن DiGA اثر مثبت درمانی را اثبات نکرده باشد.

بیمهها برای ۱۶ اپلیکیشن حذفشده در سال نخست هرکدام ۷٫۵۳ میلیون یورو پرداختند. هیچ بخشی از آن بازنمیگردد. در ۱۵ مورد از ۱۶ مورد هرگز به قیمت توافقی نرسیدند — پیش از آن ناپدید شدند.
معنایش این است: سال آزمون یک خیابان یکطرفه در هر دو جهت است. سازنده تمام خطر قیمت را به سمت پایین تحمل میکند. بیمه تمام خطر سودمندی را. هیچکس از این ساختار برنده نمیشود — و به همین دلیل نقدی که تنها یک سو را میبیند همیشه ناقص است.
قانونگذار از این چه ساخته است
از ۱ ژانویه ۲۰۲۶ دستکم ۲۰ درصد قیمت باید وابسته به نتیجه باشد. از ۱ فوریه ۲۰۲۶ تکلیف سنجش مستمر نتیجه برقرار است، در سه مرحله: از اکتبر ۲۰۲۶ دادههای استفاده همراه با نرخ رهاکردن، از آوریل ۲۰۲۸ ارزیابی خود بیماران، و از آوریل ۲۰۲۹ پرسشنامههای اختصاصی هر بیماری. REPORTED
هر دو به شکاف شواهد میپردازند. هیچکدام به عطف به ماسبق نمیپردازد.
حالا پرسشی که باید نخست میآمد: آیا این اپلیکیشنها به مردم کمک میکنند؟ پاسخ صادقانه این است — احتمالاً بله، کمی، و هیچکس دقیقاً نمیداند چقدر. این ناخرسندکننده به نظر میرسد. اما دقیقتر از هر چیزی است که سازندگان و منتقدان در ملأ عام میگویند.
اندازه اثر یعنی چه
برای مقایسه اثر، پژوهش از سنجهای به نام اندازه اثر استفاده میکند. برای فهم آن نیازی به محاسبه نیست، فقط یک قاعده سرانگشتی: ۰٫۲ کوچک، ۰٫۵ متوسط، ۰٫۸ بزرگ. ملموستر: با اندازه اثر ۰٫۲، حال یک فرد درمانشده متوسط از ۵۸ نفر از هر ۱۰۰ نفر درماننشده بهتر است. با ۰٫۸ این رقم ۷۹ از ۱۰۰ است.
اما نکته تعیینکننده خودِ عدد نیست، بلکه این است که با چه چیزی مقایسه شده.
وقتی گروه مقایسه چیزی دریافت کند، اثر چقدر کوچک میشود
اندازه اثر مداخلات روانشناختی دیجیتال، بر پایه نوع گروه کنترل.
اگر اپلیکیشنی را با فهرست انتظار مقایسه کنید — یعنی با کسانی که اصلاً چیزی نمیگیرند — خوب به نظر میرسد. اگر آن را با گروهی مقایسه کنید که چیز دیگری دریافت میکند، اثرها به یکپنجم تا یکبیستم کاهش مییابند. در کیفیت زندگی از ۰٫۳۵ تنها ۰٫۰۲ باقی میماند، یعنی عملاً هیچ.
و حالا جمله مهم: همه مطالعات پذیرش DiGA در آلمان از گروه کنترل غیرفعال استفاده میکنند. یعنی عدد مهربانتر را میسنجند. CONFIRMED
مطالعهها چقدر کوچکاند
دو جهان شواهد در یک حوزه
شمار شرکتکنندگان در مطالعات پذیرش، در مقیاس لگاریتمی.
یک بازبینی مستقل از هر ۲۳ مطالعه پذیرش مربوط به ۲۱ DiGA در اوت ۲۰۲۵ به داوری سختی رسید. همه مطالعات اثرهای متوسط تا بزرگ گزارش کردند — اما: «همه مطالعات دارای خطر سوگیری کلی بالا ارزیابی شدند.» بدون استثنا. بهویژه در سنجش پیامد (۲۱ از ۲۳ مطالعه) و بهدلیل دادههای ازدسترفته (۱۵ از ۲۳).
الگویی هم هست که باید دانست: در گروههای مداخله بهطور میانگین ۲۱٫۷ درصد کنار کشیدند، در گروههای کنترل تنها ۱۱٫۸ درصد. یعنی کسی که اپلیکیشن میگیرد تقریباً دو برابر کسی که در فهرست انتظار است کنار میکشد — و کسی که کنار میکشد اغلب سنجیده نمیشود. این نتایج را در جهتی آشنا زیبا میکند.
قویترین اثر اندازهگیریشده برای یک DiGA (اندازه اثر ۱٫۷۹) از کوچکترین مطالعه با ۵۶ شرکتکننده در برابر فهرست انتظار میآید. این دلیل دستکاری نیست. این چیزی است که آمار پیشبینی میکند: مطالعات کوچک بیشتر نوسان دارند و نتایجی که رو به بالا نوسان کنند بیشتر منتشر میشوند.
تنها مطالعهای که پرسش پول را میپرسد
تقریباً همه مطالعات میپرسند: حال مردم بهتر میشود؟ اندکی میپرسند: آیا ارزش این پول را دارد؟ برای اپلیکیشن افسردگی deprexis هر دو وجود دارد.
نتیجه، دقیق — و چرا معمولاً نادرست نقل میشود
در یک کارآزمایی تصادفیشده با ۳٬۸۰۵ بیمهشده نزد یک بیمه بزرگ، هزینههای کل در گروه اپلیکیشن ظرف یک سال ۱٬۰۲۱ یورو بهازای هر بیمار کاهش یافت (منهای ۳۲ درصد). این عدد تقریباً در هر خلاصهای هست.
آنچه حذف میشود: در گروه مقایسه هم هزینهها کاهش یافت — ۴۳۶ یورو (منهای ۱۳ درصد). پس تفاوت واقعی میان دو گروه حدود ۵۸۵ یورو است، نه ۱٬۰۲۱. این تفاوت بهسختی از نظر آماری معنادار بود (p = ۰٫۰۴).
و نکته دیگر: مطالعه اقلام جداگانه را هم بررسی کرد — دارو، بیمارستان، غرامت بیماری. هیچکدام میان دو گروه تفاوت معنادار نداشت (p = ۰٫۱۴ / ۰٫۲۵ / ۰٫۱۶). تنها جمع کل قابل اتکاست، نه این روایت که کدام قلم صرفهجویی را میسازد. CONFIRMED
یک محاسبه مدلسازی دوم و تازهتر پرسش را وارونه میکند: زیر چه قیمتی چنین اپلیکیشنی نهفقط همسنگ درمان استاندارد، بلکه برتر از آن میشود — هم ارزانتر و هم مؤثرتر؟ پاسخ: زیر ۲۰٫۳۴ یورو در هر فصل. مبلغ پرداخت میانگین امروز ۲۲۷ یورو برای ۹۰ روز است — تقریباً یازده برابر.CONFIRMED
انصاف حکم میکند محاسبه مقابل هم بیاید، و آن از حوزه بیماری دیگری است. برای اپلیکیشنهای دیجیتال در کمردرد، یک مدلسازی اقتصادی هزینهای حدود ۵٬۴۸۶ یورو برای هر سال زندگی سالمِ بهدستآمده یافت — از منظر اجتماعی، یعنی با احتساب غیبت از کار. این بسیار پایینتر از چیزی است که نظامهای سلامت معمولاً حاضرند بپردازند. با این معیار، اپلیکیشن ارزان است. هر دو محاسبه جدیاند و با اینهمه یکدیگر را نقض میکنند. دلیلش ساده و مهم است: یکی میپرسد «آیا بیمه پول صرفهجویی میکند؟» و دیگری میپرسد «آیا سودِ سلامتی ارزش قیمتش را دارد — برای جامعه؟». هر کس غیبت از کار را حساب کند به نتیجهای کاملاً متفاوت میرسد تا کسی که تنها بودجه بیمه را میبیند.
آلمان رسماً به هیچیک از این دو پرسش پاسخ نمیدهد. در روند DiGA اصلاً تحلیل اقتصادی سلامت خواسته نمیشود. در فرانسه و بریتانیا این کار استاندارد است. CONFIRMED
یک اپلیکیشن، سه کشور، سه داوری
ناپایداری زمین را یک محصول واحد نشان میدهد. deprexis در آلمان بهطور دائمی ثبت شده است. در سوئیس از ۱ ژوئیه ۲۰۲۶ بازپرداخت میشود. و NHS انگلستان در سال ۲۰۲۴ آن را ناهماهنگ با معیارهای ارزیابی خود دانست، با این استدلال که بخشهای اساسی محتوا با راهنماهای NICE همخوان نیست. CONFIRMED
یک محصول، یک مجموعه داده، سه تصمیم متضاد. هر کس ادعا کند پرسش شواهد حل شده است — در هر جهتی — آن را نخوانده است.
وقتی از «بازار DiGA» سخن میگویند، به نظر میرسد ۵۸ ارائهدهنده بر سر سهم بازار رقابت میکنند. واقعیت جور دیگری است، و تنها از آوریل ۲۰۲۶ عمومی شده است.

یک محصول واحد — برنامه چاقی Oviva Direkt — در سال ۲۰۲۵ حدود ۴۴ درصد همه کدهای تحویلشده و نزدیک ۴۰ درصد همه هزینهها را در بر گرفت. دومی، اپلیکیشن کمر Vivira، به ۵٫۹ درصد میرسد. سومی، اپلیکیشن وزوز گوش Kalmeda، به ۴٫۵ درصد. REPORTED
این رشد پرنقلقول را هم توضیح میدهد. هزینهها در ۲۰۲۵ ۶۳ درصد بالا رفت — عمدتاً بهدوش یک اپلیکیشن، در حوزهای که به دلایلی کاملاً بیربط به نرمافزار در کانون توجه است: چاقی. برای نخستین بار بیماریهای متابولیک (۳۴٫۲ درصد) از بیماریهای روانی (۲۸٫۳ درصد) بهعنوان بزرگترین گروه استفاده پیشی گرفت.
این برای بقیه چه معنایی دارد
پیامد را حساب کنیم. از ۱۷۱ میلیون یورو هزینه سالانه، حدود ۶۸ میلیون به Oviva Direkt میرسد. حدود ۱۰۳ میلیون یورو باقی میماند — برای ۵۴ اپلیکیشن دیگر. بهطور میانگین یعنی حدود ۱٫۹ میلیون یورو درآمد برای هر اپلیکیشن در سال، ناخالص، پیش از فروش، پیش از مطالعه، پیش از تأییدیه، پیش از هر مذاکره قیمت.
و «میانگین» اینجا یعنی: توزیع واقعی از این هم نامتوازنتر است، چون برخی از آن ۵۴ مورد بهمراتب بیشتر از بقیه میگیرند.
مقایسهای که آزار میدهد
شرکتی مانند CompuGroup Medical در حوزه نرمافزار مطب در یک سال بیش از هفت برابر کل چیزی گردش مالی دارد که بیمههای درمانی در همان سال برای همه اپلیکیشنهای نسخهای رویهم هزینه میکنند. Doctolib بهتنهایی حدود دو و نیم برابر. نرمافزاری که به پزشکان در کارهای اداری کمک میکند کسبوکاری بهمراتب بزرگتر از نرمافزاری است که بیماران را درمان میکند.
چرا جابهجا کردن این وضع اینقدر سخت است
کسانی که باید تجویز کنند پزشکاناند. پزشکان عمومی ۵۱ درصد همه تجویزهای DiGA را مینویسند — و ۷۲ درصد از تجویزهای آنها به بیماریهای متابولیک میرسد. در محصولات منفرد تمرکز از این هم شدیدتر است: ۸۶ درصد تجویزهای Oviva از پزشکان عمومی میآید، ۷۸ درصد تجویزهای Vivira از ارتوپدی، و ۸۶ درصد تجویزهای Kalmeda از متخصصان گوش و حلق و بینی. معنایش این است: هر اپلیکیشن به یک تخصص پزشکی واحد وابسته است. هر کس آن گروه را به دست نیاورد بازاری ندارد — و در آلمان به پزشکان از راه تبلیغات نمیرسند، بلکه از راه انجمنهای تخصصی، راهنماهای بالینی و نرمافزار مطب. این مسئله رشد نیست که با بودجه بازاریابی حل شود.
چهار یافته که تقریباً در هیچ گزارشی درباره اپلیکیشنهای نسخهای نمیآید — و ارزیابی این مدل را وارونه میکند.

سازوکاری که دیده نمیشود: هر دو جمله درستاند — همزمان، درون یک قاعده. سال آزمون خیابانی یکطرفه است در هر دو جهت. رو به پایین، سازنده تمام خطر قیمت را میپذیرد: هر آنچه در سال اول به دست آورده و خرج کرده میتواند بازپس گرفته شود، و هرچه موفقتر بوده بیشتر. رو به بالا، بیمه تمام خطر سودمندی را میپذیرد: اگر یک سال برای اپلیکیشنی بپردازد که هرگز سودمندی را اثبات نمیکند، چیزی پس نمیگیرد.

چرا این تصمیم را عوض میکند: هر کس تنها یک سو را ببیند، اصلاحی پیشنهاد میکند که سوی دیگر را بدتر میکند. سهم ۲۰ درصدی وابسته به نتیجه از سال ۲۰۲۶ فشار پرداخت بر سازندگان را بیشتر میکند، بیآنکه خطر بازپسگیری بیمهها را کم کند. اصلاح واقعی چیز سادهای است: قیمت سقفدار از روز نخست، بهجای بازمحاسبه پس از دوازده ماه. دقیقاً کاری که فرانسه و سوئیس کردند.

چرا این تصمیم را عوض میکند: پرسش درست از یک ارائهدهنده این نیست که «بازار DiGA چقدر سریع رشد میکند؟» بلکه این است: «حوزه بیماری تو چقدر بزرگ است و باید چند پزشک از یک تخصص واحد را متقاعد کنی؟» هر کس این عدد را نداند، بر پایه میانگینی برنامه میریزد که وجود ندارد.

گزارش DiGA برای ۲۰۲۰/۲۱ تا ۲۰۲۵ ارقام سالانه ۲۰، ۶۰، ۶۴، ۱۰۶ و ۱۷۱ میلیون یورو را میآورد. جمع آنها ۴۲۱ میلیون میشود. همان گزارش بهعنوان جمع تجمعی ۴۰۱ میلیون میگوید. تفاوت دقیقاً ۲۰ میلیون یورو است.

هیچ کشوری مدل آلمانی را نپذیرفت

فرانسه سقف گذاشت

سوئیس شاید جالبترین راه را برگزیده است. از ۱ ژوئیه ۲۰۲۶ اپلیکیشنهای دیجیتال رفتاردرمانی شناختی برای افسردگی بازپرداخت میشوند — و قیمت مذاکره نمیشود، بلکه با فرمولی منتشرشده محاسبه میشود. پارامترها در سندی رسمی آمده است: هزینه عملیاتی حداکثر ۲۰۰٬۰۰۰ فرانک در سال، نگهداری حداکثر ۶۰٬۰۰۰، ورود به بازار مقطوع ۲۵۰٬۰۰۰، دو فرانک برای هر خط کد، و استهلاک در سه سال.

آمریکا به پزشک میپردازد، نه به اپلیکیشن
از ژانویه ۲۰۲۵ در صورتحساب مدیکر آمریکا سه کد تازه برای دستگاههای درمان دیجیتال سلامت روان وجود دارد. این کدها زمان مدیریت پزشکی پیرامون اپلیکیشن را میپردازند — حدود ۴۰ تا ۵۲ دلار برای هر بازه ماهانه. برای خودِ محصول هیچ قیمت سراسری وجود ندارد؛ هر پیمانکار منطقهای آن را خودش تعیین میکند.
اینکه این برای ارائهدهندگان چقدر بیرحمانه است را مشهورترین پرونده این صنعت نشان میدهد. سه مهلتی که اهمیت دارند
۱۵ اکتبر ۲۰۲۶ — نخستین گزارش. از آن روز سازندگان باید برای نخستین بار گزارش کنند اپلیکیشنشان واقعاً چگونه استفاده میشود: مدت استفاده، تعداد دفعات، و نرخ رهاکردن. با این کار بزرگترین شکاف اطلاعاتی امروز بسته میشود — تا امروز دادههای استفاده نزد سازندگان بوده، نه نزد پرداختکنندگان. CONFIRMED

مشکلی که از این پدید میآید: حدود ۵۸ اپلیکیشن ثبتشده از همین حالا به زیرساخت سنجش قابل اتکا، قابل ممیزی و مستمر نیاز دارند — ثبت رویداد، تحلیل همگروهی، تعریف رهاکردن که در برابر یک بیمه دوام بیاورد، و قالب گزارشی که با طرح قانونی بخواند. بهسختی ارائهدهندهای با ۱٫۹ میلیون یورو درآمد سالانه میتواند این را درونسازمانی بسازد. و خطا گران است: هر کس نرخ رهاکردن را نادرست تعریف کند، قیمت خودش را خراب مذاکره میکند.
چرا این محصولی برای مصرفکننده نیست: مشتری سازنده است، نه بیمار. شمار مشتریان دورقمی است. قرارداد حق اشتراک سالانه است، نه قیمت بهازای استفاده. این پرزرقوبرق نیست — و دقیقاً به همین دلیل هیچ سرمایه خطرپذیری آن را تأمین نکرده است.
اندازهاش چقدر است: صادقانه بگوییم، کوچک. با ۵۸ ارائهدهنده و حق اشتراک سالانهای معقول در محدوده میانی پنجرقمی، از بازاری چند میلیون یورویی در سال سخن میگوییم — در آلمان. این پروندهای برای سرمایه خطرپذیر نیست. یک کسبوکار خوب و سودده مشاوره و نرمافزار برای تیمی پنجنفره است. هر کس آن را بهعنوان داستان رشد بفروشد، به برنامه مالی خودش دروغ میگوید.
چه چیزی مخالف آن است: کمیته ارزشگذاری و اتحادیه بیمهها میتوانند طرح گزارشدهی را چنان استاندارد کنند که به یک تیک در نرمافزارهای تحلیلی موجود تبدیل شود. ارائهدهندگان بزرگ — همانهایی که ۴۴ درصد بازار را دارند — آن را درونسازمانی میسازند. و مهلت میتواند جابهجا شود؛ در حقوق سلامت آلمان این بیشتر قاعده است تا استثنا.

آنچه همین حالا روی میز است

چه میشود: بیمهشدگان ۱۰ درصد یا ۱۰ درصد بهعلاوه ۱۵ یورو میپردازند. پیامد: نرخ فعالسازی میافتد. امروز ۸۲ درصد کد خود را فعال میکنند — با پرداخت ۲۳ تا ۳۸ یورو این نرخ باید بهطور محسوس پایین بیاید، بهویژه در درآمدهای پایین. این دو بار به ارائهدهندگان ضربه میزند: فعالسازی کمتر یعنی درآمد کمتر و دادههای استفاده بدتر، که از اکتبر ۲۰۲۶ باید گزارش شوند. چه کسی باید به این فکر کند: ارائهدهندگان باید همین امروز بدانند گروه هدفشان چقدر به قیمت حساس است. تقریباً هیچکدام نمیداند.
سناریوی ب — سقف قیمت از همان آغاز ممکن
چه میشود: قانونگذار قیمتگذاری آزاد در سال اول را با مبلغ حداکثری از روز نخست جایگزین میکند — مدل فرانسوی یا سوئیسی. پیامد: عطف به ماسبق ترسش را از دست میدهد، چون دیگر چیزی برای بازمحاسبه نمیماند. تأمین مالی اولیه که امروز ۵۴۴ یورو برای هر تجویز است از میان میرود؛ با این کار ورود به بازار برای ارائهدهندگان کمسرمایه بهمراتب سختتر میشود. برای که خوب، برای که بد: خوب برای ارائهدهندگان جاافتاده با پایگاه موجود، بد برای تازهواردان. تمیزترین اصلاح خواهد بود و همزمان همانی که شمار ارائهدهندگان را باز هم کم میکند.
سناریوی ج — همهچیز همینطور میماند منتفی نیست
چه میشود: تکالیف تازه گزارشدهی و نتیجه اثر میکنند، قیمتها با مذاکره پایینتر میروند، و چیز دیگری تغییر نمیکند. پیامد: پالایش بازار از راه فرسایش پیش میرود نه از راه اصلاح. درخواستها که در ۲۰۲۱ ۶۸ تا و در ۲۰۲۵ ۲۳ تا بودند باز هم کمتر میشوند. بازار روی مشتی محصول با اتصال روشن به یک تخصص متمرکز میشود.
در هر سه سناریو یک چیز یکسان است: شمار ارائهدهندگان کم میشود. تفاوت تنها در این است که این کار سریع و منظم رخ دهد یا کند و از راه ورشکستگی.
چگونه تشخیص دهیم گزارهای درباره اپلیکیشنهای نسخهای قابل اتکا نیست — چه از سوی ارائهدهنده باشد، چه بیمه، چه روزنامه.
«بیمهها ۲۳۴ میلیون یورو برای DiGA هزینه کردند» — بدون ذکر سال. این رقم تجمعی از ۲۰۲۰ است، نه یک رقم سالانه. چند نشریه تخصصی آن را بهعنوان هزینه سال ۲۰۲۴ نقل کردهاند. در هر رقم DiGA نخست بپرسید: تجمعی یا سالانه؟
اندازه اثر بدون ذکر گروه کنترل. در برابر فهرست انتظار تقریباً همهچیز خوب به نظر میرسد. رقم صادقانه آن است که در برابر گروه مقایسه فعال به دست آید — و آن معمولاً پنج تا بیست برابر کوچکتر است.
«اثربخشی اثباتشده در مطالعات» بهعنوان نشان کیفیت. هر ۲۳ مطالعه پذیرش بررسیشده بهطور مستقل دارای خطر سوگیری بالا ارزیابی شدند. بدون استثنا. پرسش این نیست که آیا مطالعهای هست، بلکه این است که چقدر بزرگ بوده و با چه چیزی مقایسه کرده است.
ارقام بازاری که زیرساخت و درمان را قاطی میکنند. وقتی از «بازار سلامت دیجیتال» چند میلیاردی سخن میرود، در آن نرمافزار مطب، نوبتدهی، فناوری اطلاعات بیمارستانی و زیستفناوری هست. بازار درمانِ بازپرداختشده ۱۷۱ میلیون یورو در سال است.
نرخ رشد بدون تفکیک محصول. رشد ۶۳ درصدی سال ۲۰۲۵ عمدتاً یک برنامه چاقی است. هر کس از آن به حوزه بیماری خودش تعمیم دهد، روی باد موافق دیگری حساب کرده است.
«آلمان الگوی اروپاست.» هیچ کشوری مدل قیمتگذاری آلمانی را نپذیرفته است. هر پیرو، دقیقاً همان بخشی را کنار گذاشت که اینجا ورشکستگی آفرید.
ارقام استفاده از دست سازنده بدون تعریف رهاکردن. تا اکتبر ۲۰۲۶ هیچ تعریف الزامآوری از رهاکردن وجود ندارد. هر نرخی که پیش از آن گفته شود تعریف خانگی است.
ارائهدهندهای که در پاسخ به پرسش درباره تخصص پزشکیاش عددی نمیدهد. در این صنعت ۷۸ تا ۸۶ درصد تجویزها از یک تخصص واحد میآید. هر کس این را نداند فروش ندارد، امید دارد.
اپلیکیشن با نسخه برای شما هزینهای ندارد و میتواند کمک کند — بهویژه در دوره انتظار برای یک جای درمان، که در آلمان اغلب ماهها طول میکشد. مهمترین نکته: کد را فعال کنید. ۱۸ درصد همه کدهای صادرشده هرگز فعال نمیشوند؛ بیمه بههرحال پرداخته است. و اگر اپلیکیشن پس از دو هفته فایدهای نداشت، این اطلاعاتی برای پزشک شماست — نه شکست شخصی. هیچیک از این اپلیکیشنها جایگزین رواندرمانی نیست و هیچکدام چنین ادعایی ندارد.
اگر تجویز میکنید
دانستن دو چیز ارزش دارد. نخست: برای خودِ تجویز حقالزحمهای نیست و برای پیگیری ۸٫۱۵ یورو، یک بار در هر مورد درمانی — و تنها اگر برای آن اپلیکیشن اصلاً کد صورتحساب وجود داشته باشد. دوم: برای بیشتر اپلیکیشنها شواهد ضعیفتر از آن است که تبلیغ نشان میدهد، اما صفر نیست. برای پر کردن فاصله تا یک جای درمان، جایگزین واقعبینانه معمولاً «درمان بهتر» نیست، بلکه «هیچ درمانی» است. مقایسهای که اهمیت دارد همین است.
اگر بنیانگذار یا سرمایهگذارید
کسبوکار را با قیمت توافقی حساب کنید، نه قیمت فهرست — یعنی با حدود ۲۲۷ یورو، نه ۵۴۴. فرض کنید تفاوت سال اول ممکن است عطف به ماسبق بازپس گرفته شود، و آن را کنار بگذارید بهجای اینکه صرف رشد کنید. تخصص پزشکی خود را بهنام مشخص کنید و بشمارید چند نفرند. و از خودتان همان پرسشی را بپرسید که این گزارش پیش میکشد: کسبوکار شما درمان است — یا زیرساخت پیرامون آن؟ پول تا امروز تقریباً بهطور کامل در بخش دوم درآمده است.
اگر سیاستگذار یا بیمهگرید
سه مداخله با بهترین نسبت اثر به زحمت: نخست، مبلغ حداکثری از روز نخست بهجای بازمحاسبه پس از دوازده ماه — این علت ورشکستگی را برمیدارد بیآنکه وضع بیمهها بدتر شود. دوم، یک بررسی اقتصادی سلامت در روند پذیرش؛ امروز هیچ خواسته نمیشود، حال آنکه در فرانسه و بریتانیا استاندارد است. سوم، انتشار ارزیابیها. اینکه گزارشهای بررسی عمومی نیستند نقطهای است که مطالعات مقایسهای بینالمللی سختترین نقد را به آلمان وارد میکنند — و بدون تغییر قانون هم قابل رفع است.
مدل آلمانی بهعنوان یک خدمت درمانی کار میکند و بهعنوان یک مدل کسبوکار شکست میخورد — و دلیلش یک قاعده واحد است که هیچکس بیرون از آلمان آن را نپذیرفته است.
با توجه به هر آنچه در این گزارش آمده، من موارد زیر را درست میدانم، هرکدام با درجه اطمینان خودش.
نخست، با اطمینان بالا: ایراد ساختاری عطف به ماسبق است، نه سطح قیمت. اینکه قیمتی از ۵۴۴ به ۲۲۷ یورو بیفتد تنظیمگری سخت اما عادی است. اینکه عطف به ماسبق اعمال شود و موفقتر بیشتر بازگرداند چیز دیگری است: قاعدهای که دقیقاً همان ارائهدهندگانی را مجازات میکند که نظام ادعا میکند میخواهدشان. دو ورشکستگی مستند، که یکی از آنها پرتجویزترین محصول کشور بود، رویداد تصادفی نیستند. و سند اینکه میشود جور دیگری عمل کرد در چهار کشور همسایه است.
دوم، با اطمینان بالا: بازار DiGA وجود ندارد. یک محصول هست و یک دنباله بلند. ۴۴ درصد تحویلها و ۴۰ درصد هزینهها به یک اپلیکیشن واحد میرسد. برای بقیه رویهم حدود ۱۰۳ میلیون یورو در سال میماند — بهطور میانگین ۱٫۹ میلیون برای هر ارائهدهنده، ناخالص. هر برنامه کسبوکاری که با «بازار ۶۳ درصد رشد میکند» شروع شود، بر رشد دیگری حساب کرده است.
سوم، با اطمینان متوسط: پرسش شواهد باز است — و به سود اپلیکیشنها حل خواهد شد، اما با قیمتی پایینتر. مطالعات کوچکاند، مستعد سوگیریاند و در برابر فهرست انتظار انجام شدهاند. در همان حال، بزرگترین مطالعه هزینه تفاوتی واقعی نشان میدهد، هرچند کوچکتر از آنچه معمولاً ادعا میشود — حدود ۵۸۵ یورو برای هر بیمار در سال، نه ۱٬۰۲۱ — و یک مدلسازی از منظر اجتماعی به هزینهای برای هر سال زندگی میرسد که بسیار پایینتر از آستانههای متعارف است. نتیجه من: این اپلیکیشنها احتمالاً ارزشی دارند — احتمالاً بهمراتب کمتر از ۲۲۷ یورو و بهمراتب بیشتر از صفر. آستانه ۲۰٫۳۴ یورو در فصل را بیش از حد سختگیرانه میدانم، چون تنها بودجه بیمه را میشناسد و غیبت از کار را که جامعه بار آن را میکشد نادیده میگیرد.
چهارم، با اطمینان بالا: هر کس میخواهد در این حوزه پول دربیاورد باید روی زیرساخت شرط ببندد، نه درمان. یک ارائهدهنده نوبتدهی دو و نیم برابر کل بازار بازپرداختشده گردش مالی دارد. یک تأمینکننده نرمافزار مطب هفت برابر. بزرگترین دورهای مالی آلمان در دو سال گذشته بیاستثنا به ابزارهایی برای درمانگران رسید، نه به درمانهایی برای بیماران. این وضعیتی گذرا نیست، بلکه از شیوه قیمتگذاری این نظام برمیآید: زیرساخت مذاکره میشود، درمان تحمیل میشود.
پنجم، آنچه من نمیدانم و هیچکس نمیداند: چند نفر واقعاً یک اپلیکیشن تجویزشده را تا پایان استفاده میکنند. این عدد محوری کل این حوزه است و تا امروز بهطور عمومی وجود ندارد. در ۱۵ اکتبر ۲۰۲۶ برای نخستین بار گزارش خواهد شد. من هر گزارهای درباره سودمندی این نظام — از جمله گزاره خودم — را در آن روز دوباره بررسی میکنم.
و یک جمعبندی پایانی که در بحثها کم شنیده میشود: آلمان نخستین کشور جهان بود که حق قانونی ایجاد کرد تا نرمافزاری بررسیشده به هزینه بیمه درمانی تجویز شود. این کار جسورانه بود و کار میکند — ۱٫۶ میلیون نفر چنین اپلیکیشنی را فعال کردهاند و ۴۰۱ میلیون یورو برای آن هزینه شده، که در برابر بودجه کلی بیش از ۳۰۰ میلیارد یورو در سال کمتر از یکونیم در هزار است. دعوا بر سر مبلغی بسیار کوچک با اثر نمادین بسیار بزرگ است. و این خوب است — آزمایشها باید ارزان باشند. تنها باید دست از این وانمود برداشت که آزمایش پیشاپیش قبول شده است.
این گزارش چه کاری نمیتواند بکند
گزارش DiGA شمار مطلق تجویزها برای هر اپلیکیشن را منتشر نمیکند. سهمهای Oviva Direkt، Vivira و Kalmeda از نشریات تخصصی میآید که گزارش را تحلیل کردهاند؛ مبالغ یورویی مشتقشده از آنها محاسبه شخصی است.
ارقام سالانه هزینه از ۲۰۲۰ تا ۲۰۲۵ رویهم ۴۲۱ میلیون یورو میشود، در حالی که همان گزارش ۴۰۱ میلیون را بهعنوان جمع تجمعی میگوید. اینکه کدام رقم سالانه بیش از حد بالاست از اسناد عمومی روشن نمیشود.
نرخ واقعی رهاکردن در همان ۹۰ روز در هیچ منبع عمومی وجود ندارد. آن ۸۳ درصدِ بدون نسخه بعدی نرخ رهاکردن نیست. ارقام قابل اتکا تنها با تکلیف گزارشدهی از ۱۵ اکتبر ۲۰۲۶ پدید میآید.
درآمد تکتک ارائهدهندگان قابل مستند کردن نبود. بیشتر آنها شرکتهای کوچکی با صورتهای مالی خلاصه و بدون افشای درآمدند؛ برآوردهای رایج، برآوردند.
ارقام سرمایه خطرپذیر از دادهکاوان خصوصی با تعریفهای افشانشده میآید. دو تای آنها برای آلمان در ۲۰۲۵ نهتنها در سطح بلکه در جهت با هم اختلاف دارند.
این گزاره که در فرانسه تکلیف بازپرداخت وجود ندارد بر تحلیل یک دفتر حقوقی در سال ۲۰۲۳ استوار است، نه بر متن قانون بررسیشده؛ دسترسی مستقیم به سامانه قوانین فرانسه ممکن نشد.
مبالغ دقیق بازپرداخت آمریکا برای کدهای تازه صورتحساب از یک منبع اولیه تأیید نشد؛ رقم ۴۰ تا ۵۲ دلار از دو منبع ثانویه متناقض میآید.
